Important Registration Information
Please read the following policies carefully before submitting this registration form.
1. Ability to Participate in Art Activities and Assumption of Risk
Global Art activities include, but are not limited to, the use of paints, glues, pastels, chalk pastels, pencils, wood and plaster tools, fibers such as yarn, fabric, and paper, wax, plastic bags, batik dye, incense, plaster, drawing materials, and sculpting materials.
Global Art SoCal, LLC takes all possible precautions to reduce risk and provide safe, healthy, and enjoyable experiences. You warrant that your child is able to follow directions for all activities conducted during class.
You acknowledge that risks associated with participation in class activities may exist and that you have permitted your child to attend knowing these risks and their possible consequences, including personal injury.
2. Release of Liability
As the parent or legal guardian of the registered child or children, you agree that you will not hold Global Art SoCal, LLC liable for personal injury, property damage, or loss arising from participation in class activities. You agree to release and hold harmless Global Art SoCal, LLC from liability incurred as a result of your child or children's participation in class. These terms serve as a release for yourself, volunteers, property owners, and members of your family.
3. Fees Paid
All fees paid are final.
No refunds will be provided due to a change of mind, inability to attend classes, or the return of materials.
Makeup-class credits will only be provided when the Makeup and Absence Form has been completed and the studio has been informed of the absence before the scheduled class date.
4. Monthly Fee and Card on File
A valid card on file is required for all registered students. Please contact the front office to complete the required payment setup.
Parent or Legal Guardian Acknowledgment
By selecting SUBMIT and signing or entering your initials, you confirm that you are the parent or legal guardian of the child named in this registration form, that you are at least 18 years of age, and that you have read, understood, and agreed to the terms stated above.
