  • Are you agreeable to a home visit where a volunteer will bring their dog?*
  • ABOUT YOUR HOME

  • Size of area/yard:*
  • Fencing material:
  • Does this area/yard contain any of the following?:
  • Other Features

  • Do you have a dog door?*
  • Is the dog door large enough for a large dog?
  • YOUR EXPERIENCE WITH PETS

  • I consider myself:*
  • Note: Golden Bond Rescue does not consider having had a family dog as a child as providing significant experience. We are interested in knowing your experience with dogs when you have had primary responsibility for major decisions regarding them, i.e., you decided what kind and how much food to feed, when and whether to take it to a vet and then what kind of treatment to undertake, etc.

  • Please list your current or previous dog experience for dogs you were financially responsible for as an adult. Begin with your current or most recent dog(s) and work backwards. It is acceptable to estimate dates and ages.

  • Are there any special-needs pets in your household?
  • Your Veterinarian

  • May we contact your preferred veterinarian’s office?*
  • Note: Your preferred vet is the one who has seen your pets [including cats and/or other non-canines] most recently and/or has the most complete records on them, including but not limited to vaccinations, annual exams, etc. He/she may not necessarily be your current vet.

Golden Bond Rescue will call your veterinarian’s office to ensure that you have provided at least adequate vet care to your pets. We care only about the quality of care provided – we do not inquire about billing records or other personal information.

  • Note: If the records for your pet(s) are under a different name (e.g., maiden name or name from a previous marriage) from that on this application, please give that name here.  Similarly, if there are two different surnames given for the applicants on this application, please indicate the one under whose name the records will be found:

  • THE PERFECT RETRIEVER FOR YOU

  • Which breeds would you consider?*
  • What gender dog do you prefer to adopt?*
  • Note: Golden Bond generally finds that retrievers behave like puppies until about the age of three. Some dogs are lively and curious in the house – they like to play with you and follow you around. Some dogs lie down most of the time.

  • What type of activity level are you comfortable with in a dog?*
  • Rows
  • Please rate the following canine traits according to how much they would displease you:

  • Rows
  • Are you aware that retrievers are 'people' dogs and will want to live inside with their human family?
  • Will it displease you if the dog gets up on your sofa or your bed?
  • Is being able to take your dog to a “dog park” a must?
  • Would you be willing to adopt more than one dog at a time, dogs that are inseparable?*
  • Would you be willing to adopt a senior dog (8+ years old)?*
  • Would you be willing to adopt a special needs dog?*
  • Note: By “special needs,” we mean conditions such as special diets, assistance with getting into/out of a car, behavior issues, etc.

  • Would you be willing to adopt a puppy?*
  • Would you accept a mix that is part retriever?*
  • CARING FOR YOUR RETRIEVER

  • Note: We know that many people take their dogs with them when vacationing.  The next question asks where your pet will be housed when and if a situation, e.g., an out-of-town wedding or funeral, arises in which you can’t take your dog with you.

  • When you travel, who will care for the dog?*
  • How will you exercise the retriever?*
  • What type of collar will you use?*
  • Are you aware that a retriever is a large breed and has higher maintenance costs (food, vet and grooming bills, equipment) than smaller breeds?
  • Your Retriever and Children

  • If you plan to involve children under 16 years old in the care of your dog, how much adult supervision of dog and child will be provided?*
  • With neighborhood children?*
  • How much responsibility will your child or children be given in the care and management of the dog? (Skip the next section if you checked “N/A” above.)

  • Rows
  • TRAINING YOUR RETRIEVER

  • Will you agree to take the dog to a positive-reinforcement training class or work one-on-one with a positive-rewards trainer if recommended by your Adoption Manager or the dog’s foster family?*
  • Have you ever completed a training class with a previously owned dog or current dog?
  • Do you understand that most of our rescue dogs will come with minimal training and will need a large amount of patience?
  • Note: Some of the rescued retrievers have been backyard dogs or strays and are therefore not housetrained. Some housetrained retrievers will have accidents in the house for a while after they come into an unfamiliar home.

  • Have you ever housetrained a dog before?
  • Are you comfortable with having to housetrain a dog?
  • Are you aware that the Adoption Fee for this dog(s) will be the least of your expenses during his/her lifetime?
  • Note:  Expenses include good nutrition, annual vet exams and shots, and grooming.  As with most pedigree breeds, retrievers may be prone to skin allergies, some degree of hip dysplasia and cancers that can be expensive to treat.
