BONDED BY BATTLE MMA KIDS SEMINAR
Liability Waiver, Release of Claims & Parental Consent Form
I, the undersigned parent or legal guardian of the minor child listed below, give permission for my child to participate in the Bonded By Battle MMA Kids Seminar. I understand that participation in mixed martial arts activities, including but not limited to warm-ups, drills, grappling, striking instruction, conditioning, and partner exercises, involves inherent risks of injury. These risks may include cuts, bruises, sprains, broken bones, concussions, muscle strains, serious bodily injury, permanent disability, emotional distress, or, in rare circumstances, death. I voluntarily assume all such risks on behalf of my child and consent to their participation with full knowledge of these dangers.
I certify that my child is physically fit and medically able to participate in this seminar. I understand that it is my responsibility to disclose any medical conditions, allergies, injuries, or limitations prior to participation. In consideration of my child being allowed to participate, I, on behalf of myself, my child, my heirs, and assigns, hereby release, waive, and hold harmless Bonded By Battle, its owners, instructors, coaches, staff, volunteers, sponsors, venue owners, and affiliates from any and all claims, liabilities, damages, demands, actions, or causes of action arising out of or related to participation in the seminar, including those arising from negligence, to the fullest extent permitted by law.
In the event of an emergency, and if I cannot be reached, I authorize Bonded By Battle staff or representatives to obtain emergency medical treatment for my child as deemed necessary. I also understand that my child must follow all safety instructions, gym rules, and directions given by coaches or staff members and behave respectfully at all times. Failure to comply may result in removal from the seminar without refund.
I understand and agree that Bonded By Battle may photograph or record participants during the seminar for promotional, advertising, or social media purposes unless I indicate otherwise in writing. I further agree to indemnify and hold harmless Bonded By Battle and its representatives against any claims brought by or on behalf of my child arising from participation in the seminar.
I have carefully read this agreement, fully understand its contents, and sign it voluntarily. This agreement shall be governed by the laws of the State of Texas.
