Please complete all the information on the following pages. Missing information may delay the adoption screening process. Completion of this application form does not guarantee that POP or GHOP will place one of our pets in your care. Completion of this questionnaire authorizes POP or GHOP and it's REPRESENTATIVES to verify any information contained herein, including verification of medical records of any present or prior pets treated by the veterinarians listed. Completion of this adoption process is an indication that you have read, understand and agree to abide by our Animal Adoption Agreement and that the information contained within this application is accurate and not misleading in any way. All adoptions are at the sole discretion of POP/GHOP.

Please give careful consideration to adopting a dog. Dogs are not toys or short-term commitments. Make sure your lifestyle allows the time, patience, training, and expense this dog will need over the years.

All pets will be age appropriately vetted prior to adoption. Approved adopters are expected to travel to pick up their pets. Adopters not local to the animals location, we are able to work with a local rescue near you to facilitate an adoption.

We will be speaking to all household residents to confirm everyone is aware of this application process and wants to have a dog added to the family. Please do not submit this application if you are "trying to talk someone into" adopting this dog or think "once they meet the dog, they will want one" or "I'm getting this dog as a birthday present for them." In those scenarios, you are welcome to email us to discuss the dog, but we would like all adults in the home to be involved in that email discussion.

PLEASE READ: The adoption fee covers fully vetting the dog. If you live outside of the state of the location of the dog, it will also cover a health certificate for interstate travel. Your adoption fee will be the actual cost of all of this vetting.

The parties hereto agree that the owners shall abide by the following Conditions: 1. The animal is being transfered to the adopting owner(s) with the understanding that the adopter(s) is taking possession of the animal to treat as their own. 2. The animal will be treated as a family member with loving care and affection, and will do the best to ensure the dog's saftey and well being. 3. The adult dog will be feed twice daily, puppies three times daily and have a fresh water supply at all times. 4. The dog will live inside my home and will NOT be isolated from the family. I will walk my dog on a leash or exercise in a fenced yard, which must be provided. I will NEVER let my dog run loose or roam, keep my dog chained or tied up, keep it continuously in a yard, garage, patio, balcony, pen, or leave my dog outdoors, even fenced yards when no one is home. 5. I will NOT have the dog attack-trained nor will I use it for any purpose other than companionship. I will NOT have the dog's ears cropped nor dock its tail. I will NEVER allow any physical, mental, or emotional abuse of the animal. 6. I WILL take the dog to a licenced veterinarian yearly for followup care. (Puppies will need to complete booster series on schedule) Which will include boosters, heartworm test/prevention, (if the dog originated from a state with heartworm), fecal and dental exam. 7. If your dog was tested Heartworm positive, you WILL continue the prescribed course of treatment on schedule. 8. STERILIZATION OF ANY DOG ADOPTED BEFORE THE AGE OF 9 MONTHS IS REQUIRED. THE ADOPTER AGREES TO ASSUME FULL RESPONSIBILITY FOR COMPLIANCE WITH THE PROVISIONS WHICH PERTAIN TO THE TIMELY STERILIZATION OF THE PET. If not already sterilized, I will have the dog spayed/neutered by 9 months of age. And will immediately forward proof. 9. I/We affirm that no member of my household has been convicted of an animal welfare law violation such as cruelty, neglect, abandonment, etc. 10. I/We will ensure proper licensing of the animal and will attach appropriate license, rabbies and ID tags to a non choke collar to be WORN AT ALL TIMES. I/WE will also update the registered microchip with our personal information and keep our information up to date with the microchip company. I/we will ensure compliance with all applicable local animal statutes/ordinance. 11. I/we will NOT use a choke type collar at any time, EXCEPT for training and will remove immediately after a training session. UNDER NO CONDITIONS WILL A PRONG COLLAR OR ELECTRIC SHOCK COLLAR BE ACCEPTABLE! 12. I/We are adopting the animal for myself and agree to NOT give away, sell, abandon, surrender to a shelter or trade my animal. I WILL notify the rescue WITHOUT delay, if I can no longer care for or keep my animal and agree to give the rescue reasonable time to rehome the animal or place in an approved foster home, if available. I MUST notify the rescue of any behavioral problems as soon as they may become apparent! I agree to a professional behaviorists evaluation in the event of biting or agression and any costs associated with said professional. 13. I/We agree to accept responsibility and ownership of the animal at my own risk and I release the rescue from any liability arising out of possession and ownership of my animal. I agree that I am assuming total financial responsibility for my pet as of the date of this contract. The rescue will not be held responsible for any damages or expenses incurred during my ownership of my pet. 14. I/we agree to take/provide the animal obedience training classes. 15. In the event the animal becomes lost or dies, I will immediately notify the rescue. I will also immediately notify of any changes to my contact information (address, phone number, email address). 16. The animal's known background and medical history have been discussed with me. I understand that the rescue has made no representation concerning the health, condition, training, behavior or temperament of the animal. 17. I/we agree to permit the rescue to make inquiry about and enforce any of the above conditions and requirements at anytime after adoption. This will include visits to my home every 3 months and contact with my veterinarian. I/WE UNDERSTAND THAT FAILURE TO COMPLY WITH ANY OF THE ABOVE PROVISIONS WILL RESULT IN FORFEITURE OF THE ANIMAL AND A POSSIBLE FINE OF $2500. 18. I/WE UNDERSTAND that by voluntarily signing this agreement, I am entering a legal and binding contract, breach of any term(s) of this agreement is deemed actionable.
