    I want to hear from you! I’m curious about your thoughts on Community Connection, Content Relevance, and Event Coverage. The review questions are designed to gather valuable feedback from you, the residents of Oakdale. Your input will help us create a publication that truly meets your needs and expectations. Thank you for sharing your thoughts and for being an essential part of our publication. If you have a selfie with your favorite issue or a photo of the magazine on your coffee table or countertop, we’d love to see it!
  • INSTRUCTIONS FOR COMPLETING THE FORM:

    • Use the questions to prompt your thoughts on what you want to share. Feel free to add to the information or make it your own. 
    • If you cannot complete the form in one sitting, scroll to the end of the form and click the "SAVE & CONTINUE LATER" button.  You may be prompted to set up an account. You do not need to set up a JotForm account. You may skip this step. When prompted, enter your email address. Your entries will be saved, and a link will be sent to your email. Click the link when you are ready to return and complete it.
    • When you have completed the form, simply click "SUBMIT." You will receive a confirmation email with your submission details. Once you have submitted the form, we will be in touch to let you know if we need anything else.
