This form is for Guam residents to verify if their household has been randomly pre-selected for the Household Income & Expenditures Survey. Please fill out the information below, and our HIES representative will reach out to you regarding your household's status. If your household has been selected, your participation is voluntary although very highly encouraged to ensure that the cost of living in Guam is accurately represented throughout Guam's population. If your household has not been selected, you may opt in as a potential alternate to assist this important effort. Participants who complete the survey in its entirety will receive gas and/or grocery gift cards as incentive.Any questions or concerns can be emailed to uoghies@triton.uog.edu.
