We’re so excited to welcome you to the Habitat for Humanity program officially. As part of your Habitat Partnership Agreement, this completed questionnaire and a family photo are required before your Welcome to Habitat orientation.

✅ You’ll receive 3 hours of sweat equity credit for completing it—remember to record it on your current sweat equity sheet, and we’ll approve the hours when you turn it in.

Why This Matters

The information you share will be used to create your official Habitat Family Bio. This bio will be featured on our website and shared with volunteers, donors, and sponsors who are part of your homeownership journey.

Your story helps others understand the impact of their support—and may inspire someone to give, volunteer, or apply themselves. Only share what you’re comfortable with. To review other family bios for reference, please click here.
