Kindly note each submission is only for ONE person. If you have more than one participant, please submit a single form for each person.  每次只限提交一個申請。 如果你有多個報名，請為每個人提交一張申請表格。

After submitting this online pre-enrollment form, we understand that we shall receive 2 separate emails with the enrollment details submitted by our company and we shall need to do the following steps indicated in that email in order to complete the enrollment procedure with HAFFA:
提交此網上預報表格後，我們了解到我們將收到單獨兩封包含本公司提交的報名詳細信息的電子郵件，並且我們需要按照該電子郵件中指定的步驟進行操作，以完成向HAFFA的報名程序：

1. We should receive my above pre-filled Enrollment Form in separate email (see Email 2 of 2) 
   我應該收到另外一個的電子郵件有關我預先填寫的報名表 (見 Email 2 of 2)

2. Print out the Training Enrollment Form (that entire Email 2 of 2). 
   打印此報名表。

3. Sign it with the authorized signature with company chop. 
    授權人簽名並蓋上公司印章。

4. Send out duly signed Enrollment Form together with the company cheque within 1 week by post or by hand to HAFFA (8/F, China Hong Kong Centre, 122-126 Canton Road, Tsimshatsui; Tel: 27963121), which is regarded as final submission.
   請於一週內通過郵寄或親自遞交已正式簽署蓋章的報名表和公司支票致HAFFA秘書處 (尖沙咀廣東道122-126號中港中心8樓；電話：27963121)，這將被視為最終的提交。

HAFFA will reserve a seat for you within 1 week if the class is available at the time of submission.  
如果提交此網上預報表格時仍然有空位，HAFFA將會為您的名額保留一週。

Letter of Acceptance with session details will be sent out by EMAIL Only to your assigned email address within 3 working days after the enrolment deadline, provided that your payment has been received by then.  
取錄通知書與課程詳情將於報名截止日期後3個工作日內以電子郵件形式發送至申請者所填寫的電郵地址，前提是屆時已收到您的付款。

Remark:

1. HAFFA reserves the right to accept your application or not. Personal cheque is acceptable only for individual application.

2. If the 1st Choice is not available, we will automatically transfer you to your 2nd Choice, along with phone notification. We will return the cheque to you automatically if your 2nd Choice is also not available.

3. Letter of Acceptance with session details will be sent out by EMAIL Only to your assigned email address within 3 working days after the enrolment deadline. Should you do not receive by then, please contact the HAFFA Secretariat immediately before the date of training class. Otherwise, we assume you have received it and NO refund nor change of class will be made for any absentee in this case.

4. Please note that company-sponsored applicant is NOT ELIGIBLE for the partial reimbursement of course fee. MATF Application should be made by individuals. Application from company/ organization will NOT be accepted. Please note the MATF applicant has not been granted any other form of government subsidy or financial assistance for the same course/ examination.
