You are required to read and fill out this booking form. Once submitted, I will reach out to discuss details & send a booking deposit link after.
By signing at the end of this form, you agree to the terms & conditions:
- Must be 18+ to book.
- Can NOT be pregnant or breastfeeding
- A non-refundable deposit is required to secure an appointment, and it goes towards the total cost of your service. The remaining balance is due at the end of the appointment, and it will be in CASH ONLY. We do not have a shop ATM.
DEPOSIT AMOUNT:
- $50 (for designs quoted UNDER $300).
- $100 (for designs quoted OVER $300)
- Payment plan available. Inquire if interested.
- Appointments are in a FIRST COME FIRST SERVE basis to be secured by the deposit. No deposit = appointment not secured
- If you need to reschedule or cancel, please notify atleast 24 hours before your appointment. I understand emergencies can happen at any time, all I ask is that you communicate with me.
- NO SHOWS are UNACCEPTABLE. If an appointment is no-show another deposit will be required, and the first will not be counted toward the final price. I understand emergencies can happen at any time and all I ask is for you to communicate.
- Please notify me if you're going to be late. I have a 10-minute grace period. After 10 minutes, your appointment will have to be rescheduled. Please plan accordingly.
- I will be sending a draft of the design prior to your appointment. Any design changes or revisions must be communicated at least 7 days before your appointment. A major revision or design changes contacted in less than 7 days may result in the need to rebook for a separate day.
