10th Annual 2025 Five-0 Classic Volleyball Tournament Registration Form

Registration Form for the 10th Annual Five-0 Classic Volleyball Tournament. Please fill out all fields completely and clearly.
Entry Fee: $900 per team

Each entry includes:
- Up to 12 Tournament Shirts
- Up to 12 Concert Tickets
- Up to 12 Three-Day Spectator Passes
- Additional shirts or tickets are available at $10 each
Note: Teams are financially responsible for any unsold tickets.

Player Eligibility Rules
- Players who do not participate in at least one pool play game will be ineligible for playoff or championship rounds.
- College (current or recent), international, and professional players must register in the AA/Open Division.

Corrections and/or additions to the player's Roster must be made prior to the start of the team’s first game. Changes/corrections to uniform numbers must also be completed prior to the team’s first game.

ONLY ONE NUMBER (1-99) IS ALLOWED FOR EACH PLAYER, REGARDLESS OF UNIFORM COLOR OR STYLE.

VIOLATIONS OF THE ABOVE WILL RESULT IN THE FORFEITURE OF THE SET(S) DURING WHICH THE VIOLATION OCCURRED.
