Spay/Neuter/Microchip Agreement:

Hella Dam Bitches n Queens Rescue shall be named: HDBnQ Rescue

If above named dog being adopted has NOT yet been spayed or neutered, the adoptive owner agrees to have this done by 12 months of age or before sexual maturity.

The adoption fee DOES NOT INCLUDE spay/neuter, pre-anesthesia blood testing, which if elected, is to be paid for by the adoptive owner, and no examinations of this dog are covered by the adoption fee nor will they be paid for by HDBnQ Rescue unless otherwise stated and authorized below.

In addition, I, the adoptive owner named in this contract, agree that I will have the above named dog spayed or neutered as soon as possible at a vet clinic and I agree to send proof to HDBnQ Rescue via email at Leadfoster@hdbnqrescue.org. The adopter also agrees if the surgery is done at a vet of their choice the microchip will be co-registered to HDBnQ Rescue and the adoptive owner if possible, but the default registration will be to HDBnQ Rescue. The appointment for the spay/neuter/microchip must be scheduled by adopter.

All procedures, treatments, examinations and/or vaccinations, are the financial responsibility of the adoptive owner.

As the dog is not to breed or whelped any litters resulting from an unaltered animal become the property of HDBnQ Rescue & a $10,000 fee will be assessed for the unauthorized breeding.

NOTE: If pet is not spayed/neutered and microchipped by the date designated, HDBnQ Rescue has the right to reclaim the dog and terminate this Dog Adoption Contract.
The dog will be spayed/neutered and microchipped on or before the 12th month of age.
Permanent ownership of this dog is contingent upon your compliance with this Dog Adoption Addendum.
Signature*Hella Dam Bitches n Queens Rescue Inc (rescue) must receive proof of the spay/neuter of the dog noted above within the 12th month of age. The adopter agrees to be responsible for scheduling the surgical procedure at any veterinarian of their choosing along with any and all associated veterinary costs.
Signature*Adopter agrees to provide the rescue a spay/neuter certificate signed by a licensed veterinarian within 7 business days of surgery. It is the responsibility of the adopter, not the veterinarian, to ensure that the rescue has received verification that the surgery has been performed.
Signature* Adopter understands that microchips are mandatory and injected during surgery under anesthesia and will be responsible for price of the microchip as placed by veterinary practice of their choosing along with any additional fees for placement.
Signature*Failure to comply with this agreement by the 12th month of age oof the dog unless otherwise agreed to in writing, will be considered a breach of the Contract. If the adopter fails for any reason to spay/neuter the dog within the 12th month of age, the adopter will be liable for $5,000 and agrees to pay any and all fees, including but not limited to legal fees, court costs, travel expenses, depositions, telephone calls, veterinary fees, boarding fees, etc. incurred by the rescue and their agents to enforce this contract.
Signature*Transfer of ownership or housing of this dog does not limit or in any way change any of the responsibility of the above-named adopter to spay/neuter the dog) as per this agreement.
Signature*This spay/neuter agreement is an agreement that this dog will not produce a litter of puppies either as purposely bred or by accidental breeding. If a pregnancy occurs whether purposely or accidentally, the adopter agrees to pay the additional cost for a breeding dog, which is $10,000
Signature*I Adopter agrees that if a litter of puppies is produced they will be owned by HDBnQ Rescue. HDBnQ Rescue will find homes for those puppies.
Signature*If adopter fails for any reason to spay/neuter the dog before or within the 12th month of age, the adoption contract will be withdrawn and the dog will be returned to HDBnQ Rescue effective immediately.
Signature*Adopter agrees to pay the $500 deposit which HDBnQ Rescue will refund upon verification and certification of spay/neuter surgery and no earlier.
