I, first name* last name* the undersigned, being of legal age and fully competent, wish to participate in the Dog and Cat Event hosted by Hella Dam Bitches n Queens Rescue Inc. (hereafter referred to as "HDBnQ Rescue"). I acknowledge that my participation in this event is voluntary and that I fully understand and agree to the following terms and conditions:

Assumption of Risk:
I am aware that participating in an event involving dogs and cats can be inherently dangerous, and I assume all risks associated with my participation. This includes, but is not limited to, risks of injury, bites, scratches, and zoonotic diseases (diseases that can be transmitted from animals to humans).

Medical Care:
I acknowledge that I am solely responsible for my own and my dependents health and well-being, and I agree to seek any necessary medical treatment at my own expense. I authorize HDBnQ Rescue to seek emergency medical treatment on my behalf if necessary.

Waiver of Liability:
I, for myself, my dependents, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue HDBnQ Rescue, its officers, employees, volunteers, or agents from any and all claims resulting in personal injury, accidents, or illnesses (including death) arising from, but not limited to, my participation in the event.

Indemnification:
I agree to indemnify and hold harmless HDBnQ Rescue, its officers, employees, volunteers, or agents from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, brought as a result of my involvement in the event and to reimburse them for any such expenses incurred.

Photographic Release:
I grant HDBnQ Rescue the right to take photographs and videos of me and dependents in connection with this event. I authorize HDBnQ Rescue to use, reproduce, and publish these images in any manner they see fit, including but not limited to promotional and marketing materials, websites, and social media.

Compliance with Rules:
I agree to follow all event rules and regulations as well as any instructions given by HDBnQ Rescue staff and volunteers. I understand that failure to comply with these rules may result in my immediate removal from the event.

Personal Property:
I acknowledge that HDBnQ Rescue is not responsible for any loss or damage to my personal property or my dependent(s) that may occur during the event.

I HAVE READ THIS WAIVER AND LIABILITY RELEASE, UNDERSTAND ITS TERMS, AND SIGN IT VOLUNTARILY WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
