    This agreement is for cats/kittens whom are not yet fixed but will be placed in the homes of potiential adoptive families.

  • I,   *   *   on this day, Pick a Date* agree to drop off and pick up   *      to their spay/neuter surgery. I agree to pay $500 additional deposit today and understand that it will be refunded when the surgery is complete.

    If the named above cat(s) do not complete their surgery on the date HDBnQ Rescue Provides, I understand that the deposit is forfeited. I will be obligated to schedule another surgery date at a vet of my own choosing and will be financially responsible for the new surgery.   *

  • PLEASE TAKE A MOMENT TO REVIEW HELLA DAM BITCHES N QUEENS RECUE’S FOSTER POLICIES:

    Veterinary Care Policy: During the pre-adopt period, all medical expenses and treatments are the sole responsibility of the prospective adoptive family. Please note that many shelter animals come to us with sneezing, coughing, and/or URI, and may already be on medication. All cat(s) spay/ neuter will be at the expense of Hella Dam Bitches n Queens Rescue additional medical care required [ i.e. vaccinations, flea medications, emergency care] during the pre-adopt period is solely the responsibility of the adoptive family.

    Indoor Only Policy: Hella Dam Bitches n Queens Rescue has a strict policy that cat(s) may not be let outside in an unfenced and enclosed area during the pre-adoption term.

    Return Policy: If you feel the need to return your cat(s), Hella Dam Bitches n Queens Rescue will immediately try to make arrangements to transfer your cat to a different foster. However, we cannot guarantee that this transfer will take place within a certain amount of time – it may take us time to make arrangements. 

    Release of Liability: Hella Dam Bitches n Queens Rescue makes every effort to communicate breed, age, health and behavior information about each rescue animal. We often rely on the health and behavior evaluations of the shelters before receiving an animal; As a result, we require foster families to release us of all liability resulting from any wrong or incomplete information they may receive about a particular animal.

    Cats may also act unpredictably in new environments, new situations, with new people, and when scared/stressed. This is why we suggest the first week or so to not introduce the animal to new people or new animals in the home. When incorporating a cat successfully into your home with other pets, keeping the animals separate in the home, always monitored when together, feed them in separate rooms with a door in-between is the best way to integrate animals with one another.

    Hella Dam Bitches n Queens Rescue is released of all liability for any injuries or accidents that result from their cat(s) and also accept liability for any incidents that arise out of introductions of cat(s) to third parties.

    Hella Dam Bitches n Queens Rescue does not require socialization cat(s) with people outside of their immediate family, and any decision to do so must be taken solely at the risk of the family.

    By signing the form below, you acknowledge that you have read, understood, and are agreeing to the terms and policies stated above.

  • I,   *   * understand that Hella Dam Bitches n Queens Rescue and its volunteers cannot guarantee actions or disposition of any cat(s) placed into care with me. I understand that although reasonable effort has been made to assess the disposition of each cat(s) released to Hella Dam Bitches n Queens Rescue beforehand but, being living animals, they may act unpredictably in any given situation, and that I must take necessary precautions to prevent accidents and guard against unpredicted actions by any cat(s) in my care.
