  • I,*   *   on this day,Pick a Date*    agree to drop off and pick up    to their spay/neuter surgery.

    I agree to pay $500 additional deposit today and understand it will be refunded when the surgery is complete.

    If the dog does not complete their surgery on the date HDBnQ Rescue provides, I understand that the deposit is forfeited. I will be obligated to schedule another surgery date at a vet of my own choosing and will be financially responsible for the new surgery.

  • PLEASE TAKE A MOMENT TO REVIEW HELLA DAM BITCHES N QUEENS RESCUE’S FOSTER POLICIES:

    Veterinary Care Policy: During the pre-adoption period, it is the adopter’s responsibility to cover all veterinary expenses [I.e. vaccinations, flea/tick, heartworm preventatives, etc] for the dog. For minor, non-life-threatening issues, we may allow the dog to recover naturally before seeking veterinary care. Please note that many shelter animals arrive with conditions such as kennel cough or skin irritations and are often already receiving medication. 

    Unless missed appointment as stated above and/or Spay/Neuter Adoption Agreement Addendum is signed HDBnQ Rescue assumes responsibility of spay/neuter.

    Off -Leash Policy: Hella Dam Bitches n Queens Rescue has a strict policy that rescue dogs may not be let off-leash in an unfenced area or brought to dog parks during the duration of the foster arrangement.

    Release of Liability: Hella Dam Bitches n Queens Rescue makes every effort to communicate breed, age, health and behavior information about each rescue animal. We often rely on the health and behavior evaluations of the shelters before receiving an animal; As a result, we require families to release us of all liability resulting from any wrong or incomplete information they may receive about a particular animal.

    Dogs may also act unpredictably in new environments, new situations, with new people, and when scared/stressed. This is why we suggest the first week or so to not introduce the animal to new people or new animals in the home. When incorporating a dog successfully into your home with other pets, keeping the animals separate in the home, always monitored when together, and going for on- leash pack walks is the best way to integrate animals with one another. 

    Hella Dam Bitches n Queens Rescueis released of all liability for any injuries or accidents that result from the dog and also accept liability for any incidents that arise out of introductions of rescue animals to third parties.

    Hella Dam Bitches n Queens Rescue does not require families to socialize dogs with people outside of their immediate family, and any decision to do so must be taken solely at the risk of the family.

    I,   *   * ,understand that Hella Dam Bitches n Queens Rescue and its volunteers cannot guarantee actions or disposition of the dog placed into care with me. I understand that although reasonable effort has been made to assess the disposition of each dog released to Hella Dam Bitches n Queens Rescue beforehand but, being living animals, they may act unpredictably in any given situation, and that I must take necessary precautions to prevent accidents and guard against unpredicted actions by any dog in my care.
