I agree to pay $500 additional deposit today and understand it will be refunded when the surgery is complete.

If the dog does not complete their surgery on the date HDBnQ Rescue provides, I understand that the deposit is forfeited. I will be obligated to schedule another surgery date at a vet of my own choosing and will be financially responsible for the new surgery. Signature

and understand that all pets are the property of HDBnQ Rescue and I will relinquish all custody claims and rights to ownership.

that the intention is to adopt therefore HDBnQ Rescue will not provide any necessary supplies.
to take adequate care of any dog in my care by providing nutritious food, plentiful water, and adequate shelter, as well as control of external parasites (fleas and ticks).
to immediately notify HDBnQ Rescue in the event that the dog goes missing or is injured.
I agree to always walk this dog on leash, only letting them play or walk off leash in a safe, enclosed environment.
that the dog will not be placed outside on a tie-out (in a yard, in front of a café, store etc.)
to transport the dog to a veterinarian, when necessary (HDBnQ Rescue may be able provide transportation if requested)
to administer any medications prescribed by a veterinarian or inform HDBnQ Rescue if unable to do so
not to transfer care of this animal to any other person, even a friend or family member, without the written authorization of HDBnQ Rescue.
that I am responsible for the safety of the dog in my care and for the safety of other dogs and people the dog comes into contact with.
to provide HDBnQ Rescue with at least one week’s notice if you are unable to continue caring for the dog
the dog is HDBnQ Rescue’s responsibility and may not be abandoned, given, or sold to any other person or organization
and understand that all basic medical expenses for the dog will be the signee's responsibility
and understand that signee may choose to go to a vet of their own choice and will be responsible for medical treatment to the dog
to provide medical records within 3 days of medical treatment provided during the pre-adoption period
that HDBnQ Rescue is not liable for any damage to person or property that is caused by this animal and that HDBnQ Rescue is not financially or legally responsible for medical or any other expenses incurred by this animal.
that whenever a child is in the house with the dog, the child is at risk of being accidentally knocked over, scratched, or bitten, etc.
to never under any circumstances leave a child unattended with any dog
to not leave bones/toys around the house when children are present and be cautious and observant when children are eating and the dog is in the room.
If there is a child in the household under the age of 5, the dog must be in a crate or in a separate room when the dog is eating or sleeping to prevent accidents.
that Children under the age of 13 will not walk the dog unless supervised by an adult.
to be readily available and transport my foster pet(s) to their Spay/Neuter Surgery if they are not yet fixed. (HDBnQ Rescue may be able provide transportation if requested at least 3 days prior to surgery)
to return the dog immediately at the request of an authorized HDBnQ Rescue representative(s) who will pick it up.
HDBnQ Rescue may visit the home or remove the dog from the home within 3 days of request.
This document provides for the release of liability of Hella Dam Bitches n Queens Rescue Inc. (HDBnQ Rescue) and its volunteers

PLEASE TAKE A MOMENT TO REVIEW HELLA DAM BITCHES N QUEENS RESCUE’S FOSTER POLICIES:

Veterinary Care Policy: During the pre-adoption period, it is the adopter’s responsibility to cover all veterinary expenses [I.e. vaccinations, flea/tick, heartworm preventatives, etc] for the dog. For minor, non-life-threatening issues, we may allow the dog to recover naturally before seeking veterinary care. Please note that many shelter animals arrive with conditions such as kennel cough or skin irritations and are often already receiving medication.

Unless missed appointment as stated above and/or Spay/Neuter Adoption Agreement Addendum is signed HDBnQ Rescue assumes responsibility of spay/neuter.

Off -Leash Policy: Hella Dam Bitches n Queens Rescue has a strict policy that rescue dogs may not be let off-leash in an unfenced area or brought to dog parks during the duration of the foster arrangement.

Release of Liability: Hella Dam Bitches n Queens Rescue makes every effort to communicate breed, age, health and behavior information about each rescue animal. We often rely on the health and behavior evaluations of the shelters before receiving an animal; As a result, we require families to release us of all liability resulting from any wrong or incomplete information they may receive about a particular animal.

Dogs may also act unpredictably in new environments, new situations, with new people, and when scared/stressed. This is why we suggest the first week or so to not introduce the animal to new people or new animals in the home. When incorporating a dog successfully into your home with other pets, keeping the animals separate in the home, always monitored when together, and going for on- leash pack walks is the best way to integrate animals with one another.

Hella Dam Bitches n Queens Rescueis released of all liability for any injuries or accidents that result from the dog and also accept liability for any incidents that arise out of introductions of rescue animals to third parties.

Hella Dam Bitches n Queens Rescue does not require families to socialize dogs with people outside of their immediate family, and any decision to do so must be taken solely at the risk of the family.

I understand that although reasonable effort has been made to assess the disposition of each dog released to Hella Dam Bitches n Queens Rescue beforehand but, being living animals, they may act unpredictably in any given situation, and that I must take necessary precautions to prevent accidents and guard against unpredicted actions by any dog in my care.
