Please fill out a seperate entry form for each pet.

Sign up to participate by October 24th at noon.

Create an awesome costume for your furry (or not so furry) friend. Human costumes are encouraged too!

Meet up location is current to be determined.

Judging begins promptly at 2pm, but we'd suggest getting there a little earlier if you aren't participating in the parade. Pets aren't always as patient as us humans, and the judging may start a few minutes early.

The parade will begin promptly after the judging.

I hereby grant the releasee permission to use photographs and information I've provided in this form in any of the following: web-based publications, print advertisements, social media and organization newsletters.
