Employee Referral Form

To earn a $50 Employee Referral bonus, please fill out the form with your information and provide information for the person you are referring.

You will then need to tell the person you are referring to fill out a job application and make sure they reference the bonus program in their submission.

Bonus Details:

The referral bonus is $50 and will be paid once the following three items are completed:

Your referral has completed onboarding
Your referral is assigned to a client
Your referral works a total of 30 working days

The referral bonus will be issued on the following pay period after all the items above have been completed.
