This form lets us know that your child will be leaving MindPlus.

We know that MindPlus is just one option you have for your child and our aim is always to support your child and your family.

If MindPlus is not working out well for your child, your first port of call is always to chat with your child’s MindPlus teacher. BEFORE you fill in this form, please make a time to chat and let’s see what we can do together to make things work. Financial Terms and Conditions (summary)

These terms and conditions apply to all children attending MindPlus:

Fees are non-refundable

We cannot give refunds for absences eg for illness or school events

We will provide either credit or refund in the rare event of a class cancellation

Fees are invoiced and are payable in advance before the start of each term

Weekly or fortnightly payment plans may be set up if preferred

If your child leaves during a term, you are liable for the full term fee

If your child leaves at the end of a term, you will not be invoiced for the following term

If you cannot pay the fee please let us know as soon as possible to work out an agreed and manageable payment plan

If we are not able to agree on payment plan, your child will not be able to continue at MindPlus until the debt is paid and we may need to refer the outstanding debt to a debt collection agency
