    • Why do we need for parent/caregiver details?

      The age range of our Young Neurodiversity Champions is 15-23 years. If you are under 18, we need to be able to touch base with your parent or caregiver to check that they are ok with you joining us. If you are over 18, having an extra point of contact for you is really helpful in case your phone number or email address changes.

      We'd love to know more about who you are, what you are like, and why you would like to be a Young Neurodiversity Champion.

      There are four sections here:

      2. About your experiences as a young neurodivergent person
      3. What changes you would like to see
      4. Why you would like to be a Young Neurodiversity Champion

      You can tackle each section on it's own or merge them together. We've given you lots of opportunities to share here, but don't feel that you have to fill in everything - we just want to be able to understand who you are and why you might make a great Young Neurodiversity Champion.  

      You can either write directly in this form OR upload a file, voice recording or video - whatever suits you best. If you are uploading a file, please double check it's easily shareable so we're not 'locked out' of accesing it. The max size for each file you upload is 500MB.
