INSURANCE, CERTIFICATE, AND PERMIT REQUIREMENTS

- Attach a copy of your General Liability Certificate of Insurance AND Endorsement naming Italian Festivals, Inc. as an additional insured.
  The certificate must include the date of the festa and Italian Festivals, Inc.
- Attach a copy of valid seller's permit
- Attach a copy of valid health insurance permit
- Attach copies of valid food handlers' cards

AGREEMENT

The Little Italy Heritage Festa organizers reserve the right to limit vendors and limit similarity of vendors. You must read and agree to the terms and conditions and this release of liability for your registration to be valid:

I/We hereby release (1) Italian Festivals, Inc., its board of directors, members,
volunteers, agents, and representatives; (2) Washington Elementary School Foundation; (3) San Diego Unified School District; and (4) Convivio Society from any and all liabilities whatsoever for personal injury, property damage or loss, specifically including, without limitation, injury or damage to booth contents, merchandise, employees, contractors, guests, customers, invitees, and any other third party, resulting in participation at this event.

I/We further agree to abide by all rules, regulations, and policies governing participation in this event.

Italian Festivals, Inc. reserves the right to deny any vendor application or remove any vendor from the event for noncompliance with our rules, regulations, and policies.

BY SIGNING, YOU AGREE TO THE AFOREMENTIONED TERMS AND CONDITIONS OF PARTICIPATION IN THE LITTLE ITALY HERITAGE FESTA.
