I understand that participation in the HICF Soccer Camp involves physical activity and carries a risk of injury. I voluntarily allow my child to participate and agree that Hope Int'l Christian Fellowship (HICF) Church, its volunteers, coaches, and staff shall not be held liable for any injuries, accidents, or loss that may occur during camp activities. I certify that my child is physically able to participate and will follow all instructions and safety guidelines. In case of emergency, I authorize camp staff to seek necessary medical treatment for my child if I cannot be reached.

PICTURE PERMISSION

I grant permission to HICF Church and its representatives to photograph and video record my child(ren), and to feature them in promotional materials, including social media platforms such as Facebook, for the purpose of promoting HICF Soccer Camp activities.
