    HILS “Emergency Supply Pack” Referral Form 

    Important information:

    • Deliveries will normally be made between 11.30am and 2pm throughout Hertfordshire seven days a week, 365 days a year. Please provide delivery details for places/people that can receive the Emergency Supply Pack within this time frame.
    • For same day delivery for a standard pack with long-life products, HILS must receive referrals by 10:30am

    Health & Independent Living Support (HILS) provides a chargeable “meals on wheels” service throughout Hertfordshire 7 days a week, 365 days a year.

     A chargeable breakfast service and a tea meal service are also provided countywide, seven days a week. These are delivered with the lunchtime meal and placed in the client’s fridge for eating later that day (tea) or the following morning (breakfast). NB: The breakfast and tea services are accompaniments to the main “meals on wheels” service and cannot be ordered on their own.

    Chargeable Pop-in Visits with free Medication Prompts are also available.

    We are able to provide the above services for just a few days, or longer-term.
