注意事項
1) 得獎者可依據所獲得的得獎名次，於下表選擇申請相應的比賽獎項。
2) 所有比賽獎項均會刻有得獎者的中文姓名。
3) 付款方式：銀行轉賬、轉數快或PayMe 【請於付款時備註參賽者英文姓名及聯絡電話】，上傳至表格。
4) 請於2024年5月25日或之前填妥並遞交此申請表格。逾期遞交之申請概不受理。
5) 得獎者一經申請比賽獎項，所有申請費用均一律不獲發還。
6) 奬項及獎狀將於獎項申請截止後六至八星期內派發。
7) 奬項及獎狀均採用順豐速運運費到付寄送。所有獎項一經簽收或領取，如有任何損壞，請於24小時內通知本會，否則將不獲處理。
*遞交表格前，請確保填寫的資料正確無誤。如因填寫錯誤資料導致相關獎項未能送達，本會概不負責。
Precautions
1) Make sure you have selected the corresponding prize(s).
2) All awards will be engraved with the awardee's name (Chinese).
3) Payment method: Bank transfer / FPS / PayMe (Please remark participant name (English) and contact number when making payment).
4) Submit this application form on or before 25 May, 2024. No late submission will be accepted.
5) The application fee is non-refundable.
6) All awards and certificates will be distributed within 6 to 8 weeks after the deadline for the awards application form.
7) All awards and certificates will be delivered by SF Express. Once the awards and certificates are signed or received, if there is any damage, please notify the association within 24 hours, otherwise, it will not be processed.
*Please ensure the filled information is correct and accurate before submitting the form. The Association will bear no responsibility for not receiving relevant prizes because of incorrect information.
