第六屆 舞吧！香港青年兒童舞蹈家大賽2026 - 申請表

報名流程 

1）支付報名費：Payme/Alipay/轉數快/銀行轉帳 ；

2) 在收據上/備註上，填上寫 「DANCE、參賽者中英文姓名、聯絡電話」，上傳到表格內；

3）填妥此表格，提交表格後，請查閱電郵，將收到電郵回覆（如未見請查看垃圾郵箱）；

 

*所有資料請核對清楚，如有任何因填寫錯誤，而未可把賽果、獎項等事宜，發送給參賽者，本會不會因此而負上責任。

Enrolment

1) Pay the application fee through Payme/Alipay/FPS/bank transfer.

2) Please put down "DANCE, both Chinese and English names of the applicant and the contact number" on the receipt or in the remarks, and upload it onto this application form.

3) Complete this application form. After submitting the form, please check your mailbox for the confirmation letter. (If it is not found in the mailbox, please kindly check mails in spam/junk.)

Please double-check all personal information and payment details before submission. If the results of the competition or prizes are unable to be delivered due to incorrect information provided by participants, the Association shall bear no responsibility.

個人組別 - 港幣300元/每項目
團體組別 - 港幣450元/每項目

1. 繳費證明必須保留至比賽完結
2. 請於入數證明上寫上 [DANCE、參賽者中英文姓名、聯絡電話 ]
