  • Employee Performance Evaluation Instructions

  • The evaluation form is to be completed by the supervisor independently. Supervisors will schedule a time with the employee to meet and conduct the formal evaluation. Evaluations are to be completed, signed by both parties, and forwarded to HR.

    The evaluation will be comprised of Performance Competencies (skill sets needed to perform your job) and Factor Statements (specific criteria for the competency). The comment section is available to explain the established rating by using examples of tasks from the job description.

    3 - Exceeds Expectations
    Result clearly and consistently meet or exceed goals and objectives defined for the Competency/Factor Statements for this position. Minimal direction and/or supervision is needed for this Competency/Factor Statement. Note, a rating of 3 requires a written explanation with examples from the tasks on the job description.


    2 - Meets Expectations
    Meets most if not all requirements for the Competency/Factor Statements identified for this position and generally fulfills the most essential requirements of the position. Performance on Competency/Factor Statements is competent, and supervision required is generally appropriate for the position.


    1 - Needs Improvement
    Does not meet reasonable performance expectations, improvement is necessary to meet the minimal requirements of the position on this Competency/Factor Statement. Note, a rating of 1 requires a written explanation with examples from the tasks on the job description.

     

    Each performance competency has several Factor Statements. Assign a rating 1, 2, or 3 to each Factor Statement based on the scale above. Please use the comment section to provide specific examples for a rating of either 1 or 3. A rating must be selected for each Factor Statement.

    The overal total rating will be calculated based on the average rating for each competency section.
