To ride the Bull, every senior and one parent/guardian (that is two total signatures) must read and sign the waiver found here. You can also print, sign and return this form to the Atrium desk in an envelope clearly marked "SSO Waiver". This form is due May 10, 2024.
Remember - No waiver, no bull!
Waivers will not be accepted the night of the event.

PARTICIPANT’S AGREEMENT, RELEASE OF LIABILITY & ASSUMPTION OF RISK:
I acknowledge that I and/or my child participating in activity through the rental of the contracted amusements can bring with it known and unanticipated risks to all participants. Those risks include but are not limited to physical and/or strenuous exercise or activity that can cause physical and emotional injury and distress, paralysis, death, or personal damage to my child or me. I also understand, all participation in said activity is purely voluntary, with no forcing of participation by Castle Party Planning LLC & Notre Dame de la Baie Academy, their officers, employees, agents, contractors, or representatives.
I understand that no participant will partake in said activity having any physical condition or prior injury that could result in danger to participant’s health. I further acknowledge that Castle Party Planning LLC & Notre Dame de la Baie Academy, operators/organizers of the event, at his or her discretion, may deny participation to anyone for safety reasons, including but not limited to size, behavior, medical or physical condition, intoxication, etc. I hereby expressly release, forever discharge and agree to indemnify and hold harmless, Castle Party Planning LLC & Notre Dame de la Baie Academy, their officers, employees, agents, contractors, or representatives from any loss, liability, damage claims, cost or expense which may occur as a result of any injury and/or property damage that I or my child may sustain while participating in said activity. It is further understood that this agreement, release of liability, and assumption of risk is binding upon me or my child heirs, executors, administrators, and assigns. Castle Party Planning LLC & Notre Dame de la Baie Academy, their officers, employees, agents, contractors, or representatives will be reimbursed for any reasonable attorney fees and costs to enforce this agreement. I expressly agree to indemnify and hold Castle Party Planning LLC & Notre Dame de la Baie Academy, their officers, employees, agents, contractors, or representatives harmless from all such fees and costs.
I Confirm My Understanding That:
This Agreement is intended to be as broad and inclusive as permitted by law. I agree to waive, discharge claims, and release from liability Castle Party Planning LLC & Notre Dame de la Baie Academy, their officers, employees, agents, contractors, or representatives from any and all liability on account of, or in any way resulting from injuries and damages connected to this activity to me or my child.
I have read this document in its entirety and I freely and voluntarily assume all risks of such injuries and damages and notwithstanding such risks for me and my child to participate in the activity.
I confirm that all names, signatures, addresses, and phone numbers listed below are correct and I am signing as a participant over the age of 18 years and/or as a parent or guardian of the child named below.
