I hereby authorize my employer and any individual employer covered by a Collective Bargaining Agreement with the Union, or any renewal thereof, to deduct from my wages and transmit to the Union, as Union dues, such amounts as are certified by the Union from time to time as dues check-off in accordance with the Collective Bargaining Agreement and applicable State and Federal law. This authorization shall remain in full force and effect until withdrawn by me in writing.
