By submitting this test application you acknowledge the following.

Test bookings cancelled with less than 14 day notice will be subject to a $10 re-booking fee.

Test bookings cancelled with less than 48 hours notice will forfeit the test fee, unless a medical certificate is submitted.

Skaters who do not show up for a test will forfeit the test fee.

If you are doing two tests in the same test session, you need to pay for two tests.

You arre encouraged to check your test booking at www.isq.org.au Please allow 7 days for processing.
