Fun with Math & Science 2018-2019
Request for Workshop Materials Form
If you have applied and been accepted to the FMS program for 2018-2019, please complete this materials request form once you have scheduled your first workshop and have an estimate of the number of families you plan to serve with this program.
If your first FMS workshop is in the fall and your second FMS workshop is in the spring, please complete this form twice - once in the fall with your fall numbers and again in the spring with your spring numbers.
Program Materials Request:
As part of your participation in the Fun with Math & Science program, your library will receive the following program materials and resources:
FMS booklets for preschool and elementary age children, now available in English and Spanish, may be requested at any time. These materials are free to your library and can be requested from our website at: http://libraries.idaho.gov/page/read-to-me-support-materials-order-form.
In the table below, please enter the number of families you anticipate serving at your FMS events this year. This should be a total of all families to be served - i.e. a combined number for both workshop events. If you are a returning FMS participating library, please take an inventory of any 101 Great Science Experiments books you may have left-over from the program last year, subtract these and complete the third line with a total of how many books ICfL needs to send you this year.

By applying for the Fun with Math & Science Program, your library has agreed to:
provide two or more FMS family programs anytime between September 2018 and May 2019
request program materials 3 weeks in advance of your first workshop
distribute and collect parent surveys at each workshop
provide ICfL with the completed parent surveys within 2 weeks after each workshop
complete an online summary report 2 weeks after end of overall program
After you select "Submit Form", you will automatically receive an email "Thank You" response verifying your submission, as well as an email with a pdf of your submitted request for your records.
