Deadline
- All nominations must be submitted by October 21, 2025.
- Submissions received by this date ensure the nominee receives their gift onsite at Momentum.
- Forms submitted after October 21 may result in the nominee not receiving their gift until after the conference.
Eligibility

Nominate students who have either:
- Given $500 or more to STL between Momentum 2024 and Momentum 2025, or
- Launched (not merely attended) a Youth Alive Club or Prayer Table at their school.
Honors Banquet
- Friday, October 24, 2025 at 5:15 PM during Momentum 2025.
- More details will be provided closer to the event.

Please note: if you are filling this form out AFTER 10/21 your student may not receive their gift until AFTER Momentum.

The Honors Banquet is held during Momentum 2025 on the Friday evening of the conference at 5:15pm. More details to come.
