The Nutcracker 2026 Registration Form

 

-For dancers ages 18+ please bring a headshot and dance resume to the audition. These materials are not required, but they are helpful to our staff during the casting process.

 

Audition Numbers
Audition numbers will be provided upon arrival and must be worn for the duration of the audition.

All audition numbers must be returned to staff at the end of the audition.

Measurements
All required measurements will be taken on the day of the audition by Oklahoma Ballet Theatre staff. Dancers do not need to prepare or submit measurements in advance.

Additional Notes
Dancers should arrive on time and be ready to begin promptly at their scheduled audition time. Staff will guide dancers through all necessary steps during the audition process

This fee is due upon registration and helps cover production administration, casting, rehearsal scheduling, theatre deposits, music licensing, and production planning.

Important Note: The registration fee does not include participation fees, principal role fees, or optional add-ons such as video, photography, hospitality, or merchandise. Registration reserves your dancer’s place in the audition and casting process.

Registration fees are due upon registration.

The Theatre Week Hospitality Package is designed to provide dancers with nutritious meals, snacks, and refreshments throughout technical rehearsals and performance weekend. Long rehearsal days require dancers to stay fueled, hydrated, and energized, and our hospitality team works to provide a variety of healthy and dancer-friendly options.  Each cast member, volunteer, or OBT faculty will be issued a hospitality card for meals and snacks at the theater. Single meals may also be purchased at the door. Please keep in mind that if you are filling out multiple registration forms you DO NOT need to enter multiple cards on each form.

Our official Nutcracker performance video is professionally filmed and edited by Oklahoma Ballet Theatre's official videographer. The production is captured using professional equipment and edited to create a polished keepsake of the performance for dancers and families.

Our official Nutcracker photography is provided by Emma Caster, Official Photographer for Oklahoma Ballet Theatre. Throughout rehearsals, theatre week, and performances, Emma captures professional images that preserve the memories, artistry, and excitement of the production season.Photographs may include production images, Photoshoot, performance photographs, candid backstage moments, cast photos, and featured role photography.

Celebrate your dancer, recognize a cast member, promote your business, or support Oklahoma Ballet Theatre through our official Nutcracker program.The Nutcracker program is distributed to audience members at each performance and serves as a keepsake for dancers, families, and patrons.

Oklahoma Ballet Theatre relies on parent volunteers to help make The Nutcracker possible. Families who complete an approved volunteer assignment during the production season will receive a 10% volunteer discount toward their Nutcracker participation fees.Volunteer discounts are limited to one discount per family and cannot be combined with other participation fee discounts.  If volunteer commitments are not honored Oklahoma Ballet Theatre reserves the right to recoup any discounts given throgh you online parent portal.

Liability Release

For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, as a parent or legal guardian of the above-named participant (hereinafter "Minor"), hereby grant the permission necessary to allow Minor to participate in programs, classes, rehearsals, performances, productions, events, and activities offered by Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre (hereinafter "Program"). I, in my own behalf and on behalf of the Minor, further agree to release and hold harmless Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre and any entity on whose premises the Program will occur (hereinafter the "Campus"), the affiliates of Oklahoma Youth Ballet, LLC and the Campus, and the respective directors, officers, representatives, members, agents, employees, contractors, volunteers, and affiliates of Oklahoma Youth Ballet, LLC and the Campus (hereinafter collectively "Releasees") from any and all liability for negligence or any other claim, judgment, loss, liability, cost, and expense (including, without limitation, attorneys' fees and costs) arising out of or connected with the Program, including any claim arising out of or connected with any illness or injury (minimal, serious, catastrophic, and/or death) that the Minor may incur or sustain during the Program, all activities associated with the Program, and while traveling to and from the Program whether or not the Program actually occurs. I further expressly agree to indemnify and hold harmless Releasees and Releasees' heirs, successors, assigns, executors, and administrators against loss from any further claims, demands, or actions that may subsequently be brought by the Minor or by any other person on account of damages of any character resulting to the Minor in any way from the foregoing activities. I further agree to reimburse and make good to Releasees any loss or costs Releasees may have to pay as a result of any such action, claim, or demand.

Medical Release

I acknowledge and agree, on my own behalf and on behalf of the Minor, that participation subjects the Minor to the possibility of physical illness or injury (minimal, serious, catastrophic, and/or death), and that I acknowledge the Minor is assuming the risk of such illness or injury by participating in the Program. In the event of such illness or injury, I authorize Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre to obtain necessary medical treatment for the Minor and hereby, on my own behalf and on behalf of the Minor, release and hold harmless the Releasees in exercising this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of the Minor for any illness or injury that the Minor may sustain during the Program and while traveling to and from the Program whether or not the Program actually occurs.

Appearance Agreement

I understand that from time to time Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre produces promotional material relating to its programs.

I understand that as a participant in and/or spectator at the Program, the Minor may be included in photographs, livestreams, video recordings, broadcasts, interviews, promotional materials, social media content, or other media taken during the Program.

Therefore, without reservation or limitation, I, on my own behalf and on behalf of the Minor, hereby assign, transfer, and grant to Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre, its successors, assigns, licensees, sponsors, media partners, and exhibitors the exclusive right to photograph, record, livestream, and otherwise capture the Minor and to utilize such photographs, recordings, and the Minor's name, face, likeness, voice, and appearance in advertising, promoting, documenting, and publicizing current and future programs and events.

I further understand that neither Oklahoma Youth Ballet, LLC nor any third party is under any obligation to exercise any of the foregoing rights, licenses, and privileges.

Program Rules

I further acknowledge and understand that Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre has established rules and regulations pertaining to the conduct, behavior, attendance, and activities of all Program participants, by which the Minor and I agree to abide during the Program.

The Minor and I understand that violation of the rules and regulations may result in dismissal from the Program without refund.

Refunds may be offered at the sole discretion of the Artistic Director(s) for participants who withdraw due to illness or injury. Oklahoma Youth Ballet, LLC may require documentation from a physician regarding the illness or injury for which a refund is requested.

There are no refunds for scheduling conflicts, vacations, holidays, withdrawal by choice, or similar circumstances.

Oklahoma Youth Ballet, LLC may, at its sole discretion, offer a credit for extenuating circumstances.

Registration fees are non-refundable.

Refunds and Exchanges

I hereby agree to the creation of an Oklahoma Ballet Theatre/Oklahoma Dance Center Parent Portal account in my name if one does not currently exist for the purpose of maintaining records related to my child's participation in The Nutcracker and other programs offered by Oklahoma Youth Ballet, LLC d/b/a Oklahoma Dance Center and Oklahoma Ballet Theatre.

I further agree to be responsible for all fees associated with participation, including registration fees, participation fees, principal role fees, merchandise purchases, media purchases, theatre week packages, and any other selections indicated on this registration form.

Parent Portal Account Authorization

I, on my own behalf and on behalf of the Minor, hereby warrant that I have read these Terms and Conditions in their entirety and fully understand their contents.

I acknowledge that these Terms and Conditions release the Releasees from liability and contain an acknowledgment of my voluntary and knowing acceptance of the risks associated with participation.

I understand that submission of this registration form does not guarantee casting and that all casting decisions are made at the sole discretion of Oklahoma Ballet Theatre Artistic Staff.
