St. Sophia's Forgotten Felines accepts returns of cats that were originally adopted out through our shelter at any time, for any reason. If you find yourself in a position needing to return your adopted pet, please complete the following form.

  • Timeframe for Return

    The following will help us work to find foster placement for your cat upon return.

  • Veterinary Contact & Patient Records

  • Medical records MUST be provided prior to your return appointment if you have taken your cat to the vet since adoption.

    If you do not have these records readily available, please have your vet email records to returns.stsff@gmail.com or fax to 847-739-7189. 

    NOTE: We do not accept invoices (documents showing what you paid for and how much) - we need the full patient records that show:

    • what services were performed
    • when services were performed
    • results of any diagnostics
    • veterinary notes
