Isis Central Sugar Mill may require prospective employees to undertake a pre-employment medical and drug test prior to any offer of employment being made to ensure that they:
- Are able to fulfil the inherent requirements of the position that they are being considered for, and
- Do not, by reason of their health, endanger their own or other’s health and safety.
While Isis Central Sugar Mill will protect your personal and/or health information from unnecessary disclosure we may need to release details of your personal and/or health information to Human Resources and/or management employees for the purpose of assessing your application for employment.
By submitting this application, you:
- Acknowledge that you understand that undertaking a pre-employment medical and drug test and obtaining medical clearance may be a precondition of employment with Isis Central Sugar Mill
- Consent to Isis Central Sugar Mill releasing details of your health information, including any pre-existing injury or medical condition to Human Resources personnel for the purpose of assessing your application for employment; and
- Consent to Isis Central Sugar Mill releasing details of any pre-existing injury or medical condition to the Doctor and/or physiotherapist prior to you having your pre-employment medical.
- Provide your consent for the Doctor and/or physiotherapist to release health information relevant to the position which was collected during the test to an authorised Isis Central Sugar Mill representative.
