  • Isis Central Sugar Mill Co. Ltd needs to assess whether you can perform duties that may be a requirement during your employment. The nature of the duties that you would be required to perform is included in the following Physical Requisites description. PHYSICAL REQUISITES: - Eyesight - ability to satisfy vision requirements for Queensland driver’s licence- Hearing – ability to satisfy minimum industrial workplace requirements- Ability to work in a hot, humid environment- Ability to perform work in a standing position for long periods- Must be able to lift and carry loads in accordance with the Code of Practice for Manual Tasks 2010- Ability to undertake repetitive manual handling tasks for extended periods – e.g. shovelling, raking, sweeping, stacking- Ability to walk and climb stairs and ladders regularly- Ability to bend and work with back hunched for short periods- Ability to wear appropriate PPE as needed or as advised

  • Under the Workers’ Compensation and Rehabilitation Act 2003, Isis Central Sugar Mill, with your consent, is able to obtain your claims history summary. Isis Central Sugar Mill may require you to sign an Application for Claims History Summary, which will be sent to the Workers’ Compensation Regulator. Isis Central Sugar Mill will provide you with a copy of any documentation received from the Regulator. Please ensure you read the information in the Acknowledgement and Acceptance and Declaration sections below regarding the disclosure of any information obtained by Isis Central Sugar Mill during this employment process.

    Isis Central Sugar Mill may require prospective employees to undertake a pre-employment medical and drug test prior to any offer of employment being made to ensure that they: 

    1. Are able to fulfil the inherent requirements of the position that they are being considered for, and
    2. Do not, by reason of their health, endanger their own or other’s health and safety. 

    While Isis Central Sugar Mill will protect your personal and/or health information from unnecessary disclosure we may need to release details of your personal and/or health information to Human Resources and/or management employees for the purpose of assessing your application for employment. 

    By submitting this application, you: 

    1. Acknowledge that you understand that undertaking a pre-employment medical and drug test and obtaining medical clearance may be a precondition of employment with Isis Central Sugar Mill
    2. Consent to Isis Central Sugar Mill releasing details of your health information, including any pre-existing injury or medical condition to Human Resources personnel for the purpose of assessing your application for employment; and
    3. Consent to Isis Central Sugar Mill releasing details of any pre-existing injury or medical condition to the Doctor and/or physiotherapist prior to you having your pre-employment medical.
    4. Provide your consent for the Doctor and/or physiotherapist to release health information relevant to the position which was collected during the test to an authorised Isis Central Sugar Mill representative.

    4. I authorise listed previous employers and referees to give Isis Central Sugar Mill or its designated agent any and all facts, opinions and evaluations concerning my previous employment and any other information they may have, personal or otherwise.
    5. I release all such parties from any liability that may allegedly arise from furnishing such information to Isis Central Sugar Mill including, but not limited to, any liability from defamation or invasion of privacy.
    6. I consent to a Criminal History/ Police Check based on the risks inherent with the role, and that any criminal record will be assessed against those risks.
    7. I understand that a manual drivers licence is a compulsory requirement for any role, and I understand that should I lose the ability to hold a manual driver’s licence, because of restrictions or any other reasons, it affects my ability to perform my role at ISIS Central Sugar and Isis Cane Services, and I am obliged to notify the HR department and my immediate supervisor without delay.
