MEDICAL
Isis Central Sugar Mill may require prospective employees to undertake a pre-employment medical and drug test prior to any offer of employment being made to ensure that they:

Are able to fulfil the inherent requirements of the position that they are being considered for, and

Do not, by reason of their health, endanger their own or other’s health and safety.
While Isis Central Sugar Mill will protect your personal and/or health information from unnecessary disclosure we may need to release details of your personal and/or health information to Human Resources and/or management employees for the purpose of assessing your application for employment.
By submitting this application, you:

Acknowledge that you understand that undertaking a pre-employment medical and drug test and obtaining medical clearance may be a precondition of employment with Isis Central Sugar Mill

Consent to Isis Central Sugar Mill releasing details of your health information, including any pre-existing injury or medical condition to Human Resources personnel for the purpose of assessing your application for employment; and

Consent to Isis Central Sugar Mill releasing details of any pre-existing injury or medical condition to the Doctor and/or physiotherapist prior to you having your pre-employment medical.

Provide your consent for the Doctor and/or physiotherapist to release health information relevant to the position which was collected during the test to an authorised Isis Central Sugar Mill representative.
