My Booking Intro

This booking form is for HOURLY appointments only!

This appointment option is best for flash pieces, smaller custom pieces, and anything expected to take under 3 hours.

That being said, appointment length can vary depending on size, placement, details, and how your skin takes the tattoo.

Please make sure you read my FAQ page before submitting a booking form!

I typically take up to 1 week to respond to booking requests, so please be patient with me.

Pre-Procedure Questions* This field is required.Anything about you that I need to know! *Examples: "I have fainted while being tattooed." Skin conditions (Rashes, eczema, freckles, etc.) Allergic reactions (metals, inks, creams, latex, etc.). Take any blood thinning medications (anticoagulants such as warfarin, aspirin or immuno-suppressants). Are you pregnant nursing or have diabetes, epilepsy or any blood-related diseases.

Submit Your Tattoo Request* This field is required.Please make sure everything is filled out correctly and photos are attached! Failure to correctly fill out the form will result in your submission being denied. By submitting this form, you are agreeing to all terms in my FAQ. Keep an eye on your EMAIL as that’s where all contact for booking will be directed. Unfortunately, I am not able to take on every submission and can only book so many people. If your concept is something that does not fit my current focus/style/portfolio, I will decline the proposal. Please know that this isn't personal. I am always happy to offer recommendations or other artists that may be available to fit your request.
