Student Device Use Agreement

To help ensure students have access to the tools and resources needed to engage in instruction, Southwest Public Schools (the “District”) is assigning your student a district-owned device for educational use. Signing this agreement indicates that the student and parent/guardian have read, understand, and agree to the terms below.

• The assigned device, charger, and any issued case or peripherals remain the property of Southwest Public Schools at all times.
• The device is issued for educational purposes only and is intended for the exclusive use of the assigned student.
• The device must be returned in the same condition in which it was issued, allowing for normal wear and tear.
• Changes to the device’s hardware or software are not permitted.

• Keep food and drinks away from the device.
• Never leave the device in a vehicle, outdoors, or in extreme temperatures.
• Transport the device in a padded bag or backpack compartment.
• Keep the charger with the device at all times, and report a lost or damaged charger promptly.
• Log out of or lock the device when not in use, and never share passwords or login credentials.
• Do not place stickers, decals, or other markings on the device, and do not remove district asset tags or labels. A cleaning/removal fee will be charged if this occurs.
• Do not attempt to repair, disassemble, modify, or install unauthorized software or operating systems on the device.
• Follow campus protocol for reporting device issues or damage, and seek assistance promptly rather than attempting a fix independently.
• Return the device and charger at the end of the school year or upon withdrawing from the District.

• Use of the device and the District network is governed by the District’s Acceptable Use Policy and Student Handbook, both incorporated into this agreement by reference.
• Students are expected to use the device for educational purposes, treat others with respect online, avoid cyberbullying and inappropriate content, and follow teacher and campus direction regarding use.
• In accordance with the Children’s Internet Protection Act (CIPA), the District filters and may monitor device and network activity, including use at home. There is no expectation of privacy in the use of district-owned technology.
• Violations of the Acceptable Use Policy or Student Handbook may result in disciplinary action and/or loss of device privileges, in addition to any fees described below.

• Any damage, loss, or theft must be reported to the campus technology contact within [2 school days] of discovery.
• Theft must also be reported to local law enforcement; a police report number must be provided to the campus within [5 school days] of the report being filed.
• Failure to report promptly may result in the family being held responsible for the full replacement cost, regardless of ADP enrollment.

I am purchasing the annual Accidental Damage Protection (ADP) for $25.00 from Southwest Public Schools.

• ADP covers up to one (1) incident of accidental damage to the device per school year. (Damages include: broken screen or damaged keyboard.)
• ADP is non-refundable and does not carry over to a future school year.
• ADP does not cover: (a) loss or damage that is intentional or the result of gross negligence, including failure to comply with the Student Handbook; or (b) loss or damage to peripheral items such as the charger/power adapter, battery, or case.

The following fee schedule applies to damage beyond what ADP covers and to a second incident in the same school year.

 Current fee amounts are available from the campus technology office and are set by the District independent of the ADP fee above.

 

• The device and charger must be returned by the date the campus specifies at the end of the school year, or within [3 school days] of a student withdrawing from the District.
• A device not returned by the applicable deadline will be considered lost, and the family will be charged the full replacement cost.

Outstanding fees for damage, loss, or non-return of the device may result in restriction of device replacement/loaner privileges, a hold on student records, or referral for collection, in accordance with district policy.
