COVID-19 Liability Release

I affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms of COVID-19, including, but not limited to, cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell WITHIN THE LAST 14 DAYS.

I affirm that I, as well as all household members, do not have any pending COVID-19 test results.

 
I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

 
I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 14 DAYS.

 
I understand that neither J. Blade Ranch can be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each player.

Liability Release

I take full responsibility for, RELEASE AND HOLD HARMLESS J. Blade Ranch, their owners, directors, staff, instructors, volunteers, participants, and officers from any and all liability, claims, demands or causes of action that I may hereafter have for injuries or damages arising out of my participation in J. Blade Ranch programs and events.

  I further agree that I WILL NOT SUE OR MAKE CLAIM against the Released parties for damages or other losses sustained as a result of any injury, or death, sustained from my participation in J. Blade Ranch programs and events.  I also agree to INDEMNIFY AND HOLD THE RELEASED PARTIES HARMLESS from all claims, judgments and costs including attorney’s fees, incurred in connection with any action brought as a result of participation in J. Blade Ranch programs and events activities by any of the undersigned.

The undersigned, as an applicant, parent or guardian applying for the participant or ward to use said facilities, equipment and/or programs, do hereby assume all risks and hazards incidental to the use of said equipment, facilities and/or programs either by myself or by my child or ward. I do hereby waiver, release, absolve, indemnify and agreed to hold harmless J. Blade Ranch and it’s staff or agents all except to the extent of and in the amount covered by accident or liability insurance, which insurance, however, may not be available.

I am at least 18 years of age and am competent to contract in my own name and those I have listed as minor children in my accompaniment.  I have read this release before signing below and I fully understand the contents, meaning, and impact of this release.

 

 

Media Release

I hereby grant J. Blade Ranch permission to use the participant’s likeness in a photograph and/or video in any and all of its publications, including website entries, without payment or any other consideration. I understand and agree that these materials will become the property of grant J. Blade Ranch and will not be returned. I hereby irrevocably authorize to edit, alter, copy, exhibit, publish or distribute this photo and/or video for purposes of publicizing J. Blade Ranch programs and events or for any other lawful purpose. Permission is also herby granted J. Blade Ranch to copyright such photographs and/or video-taped in its name. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein participant’s likeness appears. Additionally, I waive the right to royalties or other compensation arising or related to the use of the photograph and/or video.
