Class Application for JCAC Instructors & Designated Officials

 NOTE!

This application form is to be used only by INSTRUCTORS or other designated JCAC club officials. 

Some classes such as Distance classes, Intermediate 2, 3, Novice, and Advanced on Thursday fill up very quickly.

To enroll in the Distance classes, you must obtain the permission of the instructor.

Classes that are FULL, are labelled FULL!

If a class is labelled FULL, pls do not sign up for it.  

To enroll in Distance or Course classes, your dog must be at the Novice level or higher.

To enroll in the Distance classes, you must obtain the permission of the instructor.

Collection and Use of Personal Information

The Jumping Chollas Agility Club Inc. [JCAC] collects personal data including name, address, telephone and email information and where appropriate, birthdate for minors, from this application for the purpose of club business related to classes. The JCAC does not and will not share, publish, sell, or distribute personal information to/with other organizations without the written consent of its members and clients.

Class Fees

JCAC Members:  $100 / dog for the 1st dog; $85 / dog for subsequent dogs
Non-members:  $125 / dog
Jr. Handlers  $45 / dog
