2021 Men of Honor Weekend

Thursday, May 20th at 4:00 p.m. - Saturday, May 22nd, 2021 at 2:00 p.m.

Our host and guest minister will be Pastor Jay Eberly.

All men 13 years of age and older are invited to attend. Registration and admission payments of either $190 or $175 based on your lodging accommodation choice are required for each adult and minor attending.

Registration and 1st payment is now past due. If you are interested in attending, you must pay the full payment with your completed registration by May 2nd.

Any persons between the ages of 13 - 17 must be accompanied by an adult. If your child will be attending without you, please make arrangements for someone to chaperone your child.

Required Orientation and Safety Meeting on Wednesday, May 12th at 8:45 p.m.

For individuals or groups attending outside of the Cedar Rapids area, you will receive a recording of this meeting on Thursday, May 13th.

All skill levels will be able to participate in the Six Man Team Competition. Each activity will take strategy, strength and teamwork.

The Bearded Man Competition will be based on individual strategy, skill and strength.

In consideration for being accepted by {participantsName} (participant) for participation in Men of Honor. I, being eighteen years of age or older, due hereby release, for every discharge and agree to hold harmless Men of Honor Group and Spirit of Faith Family Church and the directors thereof from any liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersign participant that occur while said participant is participating in above described trip or activity. Furthermore, I hereby assume all risks of personal injury, sickness, death, damage and expenses as a result of participation in recreation and work activities involve therein. Further, authorization and permission is hereby given to said church to furnish any necessary transportation, food and lodging for myself (the undersigned participant). The undersign further hereby agree to hold harmless and indemnify said church, its directors, employees and agents, for any liability sustained by said church as a result of negligence, willful or intentional acts of said participant, including expenses incurred attendant thereto. I hereby grant my permission to participate fully in said trip, and hereby give my permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limitation participant to return home due to medical reasons, disciplinary action or otherwise, I hereby assume all transportation cost. If a dispute over this agreement or any claim damages arises, I agree to resolve the matter through a mutually acceptable arbitration process. Refunds: all deposits are non-refundable; non-transferable.

Your signature is confirmation that you have read and accept this policy. This form must be completed in order to process any applications. 

I agree to release, hold harmless and indemnify the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center, on behalf of myself and my family members, partners, heirs and assigns as follows:

1. I acknowledge that engaging in or receiving instructions in any activities, including but not limited to firearms, archery equipment, sports activities of any kind, zip lines and other challenge courses may involve dangers and risk of bodily injury.
2. I acknowledge that participating in any activities at camp poses risks that could result in physical or emotional injury, paralysis, death, or injury or damage to me as well as to property, or to third parties, including, but not limited to, head, neck, and back injuries. I understand that these risks cannot be eliminated without jeopardizing the essential character and quality of the activity.
3. I acknowledge that I have been fully informed of the inherent hazards and risks associated with activities in which I am about to engage, including but not limited to:
   • Inclement weather, lighting, variances and extremes of wind, weather and temperature;
   • My sense of balance, physical condition, ability to ride and hold on or follow directions;
   • The presence of insects and flying life forms;
   • Equipment failure or operator error;
   • Heat or sun related injuries or illnesses, including sunburn, sun stroke or dehydration;
   • Fatigue, chill and/or reaction time and increased risk of an accident
4. I accept and assume all the risks of participation. My participation is purely voluntary, and I voluntarily elect to participate, and I am fully aware of the risks.
5. I hereby release the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network, Sunstream Retreat Center, and their respective officers, directors, employees, representatives, agents, and volunteers from any and all liability and responsibility whatsoever and for any and all claims or causes of action that I, my estate, heirs, executors, or assigns may have for personal injury, property damage, or wrongful death arising from the above activities whether caused by active or passive negligence of those hereby released or otherwise. By executing this document, I agree to hold the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center harmless and indemnify them in conjunction with any injury, property damage, loss of life, or other potential criminal or civil liability that may occur as a result of my engagement in activities.
6. I have adequate and appropriate Insurance to cover any injury or damage I may cause or suffer while participating, or I agree to assume and bear the costs of any injury or damage.

By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center on the basis of any claim from which I have released them. I have had sufficient opportunity to read this agreement. I have read and understood it, and agree to be bound by its terms.

The intention of this form is to provide consent for a minor child to participate in this event and be in the care and responsibility of an approved adult over the age of 18. If neither parent is traveling or attending the event with the minor child, then this Parental Consent Form must be completed and signed by the minor’s parent or guardian.

We (I), being twenty-one years of age or older, due for ourselves (myself) (and for and on behalf of my child/participant, if said child is not twenty-one years of age or older) due hereby release, forever discharge and agree to hold harmless Men of Honor and Spirit of Faith Family Church and the directors thereof from any liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersign and the child/participant that occur while said child is participating in above described trip or activity. Furthermore, we (I) (and on behalf of our (my) child/participant if under the age of 21 years) hereby assume all risks of personal injury, sickness, death, damage and expenses as a result of participation in recreation and work activities involved therein. Further, authorization and permission is hereby given to said church to furnish any necessary transportation, food and lodging for this participant, and we (I) understand that our (my) child/participant will be in a lodging overnight with not less than three (3) other participants (under the age of 19 years) and adults (19 years or older) The undersign further hereby agree to hold harmless and indemnify said church, its directors, employees and agents, for any liability sustained by said church as a result of negligence, willful or intentional acts of said participant, including expenses incurred attendant thereto. We (I) are parent(s) or legal guardian(s) of this participant, and hereby grantor (my) permission for him to participate fully in said trip, and hereby give our (my) permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limitation participant to return home due to medical reasons, disciplinary action or otherwise, we (I) hereby assume all transportation cost. I do hereby give my consent for my child, {minorsName} (name), to travel to and from and to participate in the following (please check all that apply):

I, {minorsName}, an individual under the age of 18 years, desire to participate in the Event and its activities conducted by Spirit of Faith Family Church, Inc. (“Church”) at Sunstream Retreat Center. I volunteer to participate by my own free will. I agree that as a participant, I will behave in an appropriate manner and shall exercise reasonable care in participating in order to avoid damaging the reputation of or risking the assessment of liability against Church.

I agree to release, hold harmless and indemnify the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center, on behalf of myself and my family members, partners, heirs and assigns as follows:

Parent's or Guardian's Additional Indemnification (Must be completed or participants under the age of 18) In consideration of {name} ("Minor") being permitted by the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center to participate in the above mentioned activity, I further agree to indemnify and hold harmless the Iowa District Council of the Assemblies of God d/b/a the Iowa Ministry Network and Sunstream Retreat Center from all claims which may be brought by or on behalf of Minor and which are in any way connected with Minor's use of participation in the activity. I am fully informed as to the dangers and risks related to the activity and agree as the Parent or Guardian of the Minor to assume responsibility for any injury, property damage, loss of life, or other potential criminal or civil liability that may occur as a result of Minor engaging in the activities. I have full authority to sign this agreement on behalf of Minor.
