    (RRRT)

    This assessment evaluates the quality of patient handoffs at your organization, identifying strengths and areas for improvement.

    It's built on the idea that every handoff is like a relay race: success depends on the entire team, including clinicians and patients, working together to pass the baton safely.

    The RRRT™ consists of two parts:

    1) The Hand-Off Foundations Scale (HFS) measures the structural side of your handoffs: whether they follow best-practice protocols based on the five components of effective handoffs.

    2) The Hand-Off Teamwork Scale (HTS) measures the people side: the quality of teamwork, support, and trust that make those protocols work.

    This assessment is designed for healthcare professionals, including frontline clinicians, administrators, and support staff. It is free and takes less than 10 minutes to complete.

    You will receive a score for each scale, presented as a percentage. We will also send you a personalized Certificate of Completion via email.

  • Part 1

    Instructions: The Hand-Off Foundations Scale measures the structure of handoffs at your organization. Indicate how much you agree or disagree with the following statements, based on what typically happens at your organization. Some items may apply to different types of handoffs (e.g., shift-change, transfer-of-care). Answer based on whichever type is most relevant to that action. If you are not sure about a specific action, select “Neutral or Unknown.”

  • This first set of questions refers to the behaviors of “sending” clinicians or care teams, the clinicians that are handing off a patient to someone else. Indicate whether the sending clinicians at your organization perform the following actions.*
