Please complete all fields of this application.  There is an $80 vendor fee that must be paid to be considered a vendor. In addition to the payment option below, you can also use Venmo (lovejonescreations), Cash App ($lovejonescreations), or Zelle (469-993-5854) .

Once you have filled out this form and paid the vendor fee, you will be sent an informational and confirmation email. Your space is not secured until the vendor fee has been paid in full.  Please note, we will not hold any spaces or accept partial payments.

This agreement is to verify your participation in the Wellness In Motion Health and Wellness Expo on Saturday, April 15th, 2023 at the The West End Square-607 Corbin Street, Dallas, TX 75202.

Each Small Business vendor must bring own 6-foot table,Tents, 2 Chairs, Racks, Stands, etc. a 10 by 10 space that will be provided. (This is an outdoor event)

Outlets are available for electronic equipment, but are limited, those needs should be specified at the bottom of the agreement. Vendors are advised to keep products and exhibits to an 10’x10’ display size. 

Love Jones Creations and Eudaimonia Wellnes & Beauty is not liable or responsible for any accidents or incidents that occur while vendors, volunteers or patrons are in the expo area. In addition, Love Jones Creations and Eudaimonia Wellnes & Beauty assumes no liability or responsibility for any effects small business vendor products may have on patrons. Each vendor holds responsibility and liability for their own products and product effects.

Vendors may have access to event space beginning 9:00 am. The event concludes at 2:00 pm and vendors have until 3:00 pm for break down. Participation in the April 15th, 2023 Wellness In Motion Health & Wellness Expo is voluntary and vendors reserve all rights to their products and services as well as payment received for these products and services.

Vendors must pay the registration fee of $80 before vending space is reserved. Vendor spots are chosen on a first come first serve basis. Thanks to the layout of the West End Square all vending areas have an equal amount of visibility. There are no bad spaces. Show management pre selects vendor space for each small business based on your products, however, vendors reserve the right to request a different space, although it is not guaranteed.

Payment shall be made directly through stripe, cash app, or zelle. Other forms of payments are also accepted please contact event coordinator with payment questions.

Registration fees are non-refundable. In the event of a proven accident, riot, strike, epidemic, an act of God or any other legitimate condition or occurrence beyond their respective control, the Wellness In Motion Health & Wellnes Expo will be rescheduled for a date to be determined. 

Small business vendors are entitled to use the name, likenesses, and other promotional materials provided for the Wellnes in Motion A Health & Welnnes Expo. As a registered vendor you are welcome to submit a promotional item, i.e., coupon, business card, flyer etc., to be included in our complimentary registration bags given to all attendees. Your signature verifies your consent to these terms.
