2025-2026 Parent/Player Agreement

This agreement represents a monthly payment commitment that will begin upon execution and remain in effect through March 31, 2026 with the exception of players born 2018,2019,2020,2021 and players who are identified as "practice players" by the coach and management. The 12 month contract does not apply to our younger aged groups and "practice players."

Important Terms:

Monthly payment will be $180 plus $6 credit card transaction fee.
This agreement is non-cancellable.
All payments are non-refundable, even in the event of sickness, injury, vacation, or weather-related cancellations.
Payments are made on a monthly basis.
The agreement does not include Fall Season fees, which will be billed separately (CSL players will pay $400, TFA players will pay $500 due 5/1/2025)
NO Sibling discount.
Additional fees will apply if the team(s) is/are registered for Elite Academy or National Premier League.

Waiver & Assumption of Risk Waiver of Liability

In consideration of being permitted to participate in any way in a Jewel City United. activity, I, for myself, my heirs, personal representatives or assigns, do hereby release, waive, discharge, and covenant not to sue Jewel City United, its officers, employees, and/or agents, as well as all facilities where the player and/or team trains and/or plays games, including the field facility owner or operator, their respective faculty, administration, and trustees, from liability for any and all claims, including the negligence of Jewel City United, its officers, employees, and/or agents, resulting in personal injury, accidents, illness (including death), and/or property loss arising from, but not limited to, participation in Jewel City United and activities.

Assumption of Risk

Participation in Jewel City United activities carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from a) minor injuries such as scratches, bruises, and sprains to b) major injuries such as eye injuries or loss of sight, joint or back injuries, broken bones, heart attacks, and concussions, etc., to c) catastrophic injuries including paralysis and death.
I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in Jewel City United activities. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. The undersigned further expressly agrees that the foregoing waiver and assumption of risks is intended to be as broad and inclusive as is permitted by the law of the State of California, and that if any portion thereof is found invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

RELEASE OF CLAIMS

As a condition of my child being permitted to participate in the Jewel City United Program, and for and in consideration of the services provided by Jewel City United and Jewel City United’s staff, I hereby waive and release any claims that I or we may have against Jewel City United, or their trustees, directors, officers, employees, volunteers, or agents based on any loss, illness or injury (including death) that my child may sustain arising from, in connection with, or incidental to my child’s participation, whether such loss, illness or injury is caused by my student’s own acts or omissions or by those of other program participants, Jewel City United staff or volunteers.

WITHDRAWAL FROM PROGRAM

If I choose to withdraw my child from the program, I understand and agree that all fees must be paid in full prior to withdrawing, including but not limited to club fees, team fees, and any outstanding Fall Season fees.
All monthly payments must be paid up until March 31, 2026, even if I choose to leave the club before this date. This is a binding contract that will expire on March 31, 2026.
If I choose to leave the program before the contract expiration date, I authorize Jewel City United to charge my credit card on file for the remainder of the months of my contract, including any outstanding Fall Season fees.
I have read and understand the Assumption of Risk and Release. I further understand and agree that the foregoing Assumption and Release is intended to be enforceable to the fullest extent permitted by law.
The club schedules a two-week break in both the summer and winter, but members are still expected to make full monthly payments during these months.

Release Authorization For Emergency Medical Treatment:

I understand that I am required to maintain and carry accident medical insurance coverage for the child listed on this application and I verify that the coverage information attached herewith is accurate and true. In the case of an emergency, and I cannot be reached, I authorize the staff and/or agents of Jewel City United to obtain whatever medical treatment they deem necessary for the welfare of my child listed on this application. I further understand that I will be financially responsible for all charges and fees incurred in the rendering of said emergency treatment, regardless of whether my medical insurance would cover such charges and fees.

Medical Release & Insurance Release Authorization For Emergency Medical Treatment

Financial Commitment

Each player registering with Jewel City United assumes a two-part financial obligation: payment of club registration fees and payment of Team Account fees. Club registration fees include CLUB SOCCER registration fees, insurance, coaching salaries, facility rental fees, league registration. Team Account fees include team entry fees to various tournaments as determined by the team’s coach andial Commitment coaches’ travel reimbursement costs, and miscellaneous team expenses.

Late payments

Any parent failing to pay their registration fees due shall be assessed a $35 late fee five days after the payment due date. Should a player fall in arrears for either Club registration fee payments or Team Account payments, the player may be suspended from play until all accounts are brought current. Parents in arrears in excess of 30 days may be subject to involuntary removal from the club.

Refunds

The Club incurs significant nonrefundable expenses when you commit to playing for a team for a season, and thus we have a NO REFUND policy for all club fees. Exceptions may be made in the case the player suffers a season-ending injury and will be decided on a case-by-case basis by the administrative staff in consultation with the player’s coach and the Director of Coaching. You understand and agree that the installment payment plan option is not a subscription for services; it is an optional payment plan to help families spread out the club dues. You are signing up for a year commitment regardless of how you choose to pay the required club fees. Should you choose to leave the Club prior to the end of your installment payment plan being fulfilled, the Club reserves the right to immediately charge that outstanding balance to your credit card. You further understand and agree that, in the event your account becomes past due for more than 10 days, Jewel City United reserves the right to immediately charge that past-due balance to your credit card. Team Account fees are not refundable under any circumstances.

Concussion and Injury Policy

Any player showing signs or symptoms characteristic of a concussion will be removed from participation/competition. If an athlete who has been removed from athletic activity due to a suspected concussion or other physical injury, the athlete shall not return to athletic activity until the athlete has been evaluated by a licensed health care provider, and the athlete has received written clearance to return to play from the evaluating health care provider. Athlete must submit written clearance to return to play by the physician to the Coaching Director before he/she is able to return to play or practice.

TFA Glendale Chapter (if applicable)

Qualification for Total Futbol Academy Glendale Chapter (TFA Glendale)

Certain teams registered with Jewel City United may qualify to play for the Total Futbol Academy Glendale Chapter (TFA Glendale). In the event that a player or team is qualified, they must sign and abide by the rules and regulations of TFA Glendale. Failure to comply with these rules and regulations may result in disciplinary actions, including but not limited to disqualification from participation in TFA Glendale activities.
