    You are being asked to participate in a research study. This consent form will give you information about the study to help you decide whether you want to participate. It is your choice whether you want to be in this research study. Please read this form, and ask any questions you have, before agreeing to be in the study. This study asks you to complete a 4 question survey that will take no more than 25 minutes to complete. The purpose of this study is to determine the top indicators that recent graduates of art education (with licensure) programs were underprepared for and learn whether this will impact their ability to teach or to continue to teach once they have received their licensure. We are asking you to be a part of this study because you have completed your student teaching experience in art education at some point between Fall 2019 and Spring 2024. This study is being conducted by Joey Hatch, Indiana University Curriculum and Instruction Doctoral Student under the supervision of Kevin Tavin. If you agree to participate in this study, you will be asked to complete an online survey consisting of five questions about your student teaching experience and your ability to complete the experience. These questions will ask you about your previous course work, your cooperating teacher, and the supports you utilized, and or required during your student teaching experience. You can skip any questions you are uncomfortable answering. Before agreeing to participate please consider the potential risks and benefits to taking part in this study.  Anytime a person shares information online there are associated risks. To keep your information secure, this survey is being conducted online using the secure platform JotForm to collect your answers. We cannot eliminate all risks; however. All possible identifiers will be removed after the survey is completed and will be replaced with a Study ID. All electronic data will be downloaded from JotForm and stored on a portable hard drive that will be kept in a secure location. Any identifiable information retained will be kept separate from the research data collected. We will destroy the survey and the account associated with the survey after we have finished collecting and analyzing the data. You may be uncomfortable while answering the survey questions. While completing the survey, you may skip any questions that make you uncomfortable or that you do not want to answer. You may not have any personal benefits from taking part in this study. The benefits may help others in the future.  Your answers may help researchers and educators to better understand the impacts of  art education student teaching completed during/post COVID-19 pandemic. A more thorough understanding may allow for changes to be made to educational programs which will aid future student teachers of art education to be better prepared or receive adequate supports to ensure that the experience is successful and encourages recent graduates to go on to have a career in art education. You will not be paid for participating in this study. Your personal information may be shared outside the research study if required by law. We also may need to share your research records with other groups for quality assurance or data analysis. These groups include the Indiana University Institutional Review Board or its designees, and state or federal agencies who may need to access the research records (as allowed by law.) If you decide to withdraw, simply exit out of the survey, and nothing you have previously written will be collected. Please print or save this screen if you wish to access this information later.
