  Equipment
  
  For listed activities, Sports Carnival event team will provide sports equipment (ie balls, cones, etc.), Activity Host organization is asked to provide tables and chairs. If you have none, then we will work with you to provide tables and chairs. All other equipment is to be provided by activity host.
  
  Terms and Conditions:
  
  With the submission of this proposal, I agree to the following terms and conditions;
  
  * to donate prize (company shirts, kids prize, or gift basket) to be raffled at end of event.
  
  * to provide staff and all supplies outside of what is being provided by Sports Carnival.
  
  * to arrive and set up between the hours of 12pm-2pm on Sunday, July 1, 2018 and staff and conduct activity from 2pm-5pm
  
  * to understand that a canopy will not be provided by Sports Carnival, and if my organization should bring a canopy, it must be secured in the ground for the duration of the event.
  
  Liability Waiver:
  
  Sports and recreation activities present certain inherent risks and hazards, which the player-participant and adult player or parent/guardian are urged to consider and which the Player assumes. By submitting this form, on behalf of the directors of my organization, I agree to hold harmless iBall United, the Borough of Pottstown and all parties involved for any and all claims and understand and acknowledge that such recreational activities have inherent risks, dangers and hazards, foreseeable and unforeseeable, that may result in injury, illness, or property damage, or in extreme cases, death, and on behalf of myself, my family, agents and contractors, I hereby release and agree to hold harmless International Ballplayers United, Inc., its partners, sponsors, other affiliates, volunteers, coaches, managers, officers and directors, from all claims, actions, or losses related thereto. International Ballplayers United, Inc. assumes no liability for injury or damage arising from the results of participation of the above named event unless due to willful fault or gross negligence on the part of International Ballplayers United, Inc.
- What is the Sports Carnival?
  
  The 2018 Sports Carnival is a community event to provide sports and recreational activities for youth and families of the Pottstown and surrounding areas.In addition to sports and recreation activities, there is also sports clinics, awards and prizes, food, music, and a basketball tournament taking place throughout the day.

  What are the goals of the 2018 Sports Carnival?
  
  The Sports Carnival event is produced to do the following:
  *Provide healthy competition and family fun through sports, recreation, and other physical activity for youth and families of the Pottstown and surrounding communities.
  * Lead in the effort to continue the return of the Pottstown July 4th festivities which historically drew 12,000+ youth and families to Pottstown, PA.
  * Promote community unity by bringing together various area sectors and forging relationships among area residents.
  * Award prizes to youth and families who are most active throughout the day’s events.
  * Connect youth and families to vital area youth development clubs as well as other services and organizations.
