• Appointment Request Form

    Thank you for choosing me and for your interest in my art! Please provide me with a description of your design in as much detail as possible, so I am able to give you an accurate quote. You will typically receive a response from me within 48 hours of submitting your form. Please be sure to respond to any questions that I may have, via email or phone. Once you have received confirmation from me about your appointment request, you may proceed with booking your appointment through my booking link. You will fill out a consent form in person, upon arrival to your appointment. YOU MUST BRING PHYSICAL ID WITH YOU to your appointment so it can be scanned in and copied.

  • If the appointment type chosen was Touch Up, please describe the tattoo that needs to be touched up (whether it is my work, or another artist's). For the following questions about size and placement, you can give an estimate of the size of the tattoo and do your best to accurately describe where it is placed on your body. A photo upload is required if you are choosing this service so that I can see the tattoo before appointment approval. Thank you!
