I understand that physical activity involves inherent risks, and I agree that the Pretty Fit Wellness LLC, Tampa Bay Titans organization, its affiliates, staff, and volunteers are not liable for any injuries or damages that may occur as a result of my participation. I accept full responsibility for my own health and safety.

I give permission to Pretty Fit Wellness LLC and Tampa Bay Titans to use photos and videos of me taken during participation for marketing and promotional purposes (including social media and website use).

I understand I will not receive compensation and release the organization from any claims related to this use.

If I do not wish to be recorded, I will notify staff in writing prior to participation.
