Become part of history and help raise funds for Jordan and Josh Mannen's Eagle project by buying an engraved paver. All engraved pavers will be permanently installed on the grounds surrounding the flag pole at the entrance of the Prescott Vederans Hospital. All pavers need to correlate with a veteran or a military branch. 

OPTION 1 - Engraved paver is formatted in ALL CAPS and is centered on each brick.
4×8 paver will include 15 characters per line; 3 lines total. 

OPTION 2 - Engraved paver is formatted in ALL CAPS and centered on each paver.
4×8 paver will include 13 characters per line; 3 lines total, with the military branch letters listed on the right side of the paver. 

Your donation of $125.00 allows you to honor your branch or a loved one by engaging a paver as a lasting record of your valued support.

Please type in block letters (CAPITALS) and always use the ampersand symbol "&".   A character represents a letter, number, punctuation mark or space,  Max characters per line is 13 -15  (Depending on the option you choose), three lines per paver. 

Simply fill out this form, when you submit it, a copy will go to the Scouts and you will be linked back to the Spotfund account where you can make the donation. If you are doing multiple pavers you can fill out multiple Jotforms (1 form for each paver) and add your pavers donation up and run your credit card once on the Spotfund page.
