- I will inform the Club /Coaches of any important changes to my child’s health, medication or needs and also of any changes to our address or phone numbers given above. In the event of illness, having parental responsibility for the above named child, I give permission for medical treatment to be administered where considered necessary by a nominated first aider, or by suitably qualified medical practitioners. If I cannot be contacted and my child should require emergency hospital treatment, I authorise a qualified medical practitioner to provide emergency treatment or medication.
- Player Registration is not complete until this form has been Signed and Submitted. Player fees should be paid in advance via bank transfer or pay as you play prior to a coaching session following the completion of your free trial session. By signing this form you agree for KEF soccer school to use photography and video of your child solely within its websites and promotional materials. Where images are used, children's names will not be identified. Should safeguarding issues exists or you would request your child image not be used please indicate this below.
