Release of Liability and Hold Harmless Agreement

I understand that participating in Amtgard Live Action Roleplay ("Amtgard") activities involves dangers and risk, both known and unknown. I am assuming all responsibility for and all risk of property damage or personal injury that may occur to me as a participant in Amtgard; while attending meetings, events, fighter practices, or other gatherings; using Amtgard equipment or facilities; or training or preparing for Amtgard activities; in or out of an Amtgard setting. I am entering into this Agreement freely and voluntarily.

In consideration of and for the opportunity of being permitted to participate in Amtgard activities provided by or through Amtgard Polaris, Inc.; I hereby, for myself and my representatives, heirs, successors and assigns, release, terminate and discharge Amtgard Polaris, Inc. and their directors, officers, employees and their successors, assigns, officers, directors, shareholders, employees, heirs and agents (the "Released Parties") of and from any and all claims, demands, liabilities, obligations, rights to indemnification, contract rights, damages, costs, expenses, actions and causes of actions ("Claims") which I may have for any property damage or personal injury, including death, arising out of, or in any way relating to my participation in Amtgard activities through Amtgard Polaris, Inc. and all activities in any way related to such participation. This Release includes property damage or personal injury that may be foreseeable or that is entirely unforeseeable.

Participant Rules and Agreement

In order to have promote the best possible environment for Amtgard and its participants, AMTGARD POLARIS requires strict compliance with the following rules. Failure to abide by these rules may result in a Participant's removal from an AMTGARD POLARIS meeting or event, without refund when applicable: 1. I agree to follow Amtgard International's Amtgard Rules of Play, the Kingdom of Polaris Corpora, and all local/federal laws. 2. I agree to comply with any additional rules that may hereafter be communicated, whether written or orally, by Amtgard Polaris. 3.I acknowledge that at public meetings or private events there is no expectation of privacy and photos or video may be taken of me for advertising purposes. I acknowledge thatI can always contact the Amtgard Polaris Board of Directors to request my image not be used and the Board will review that request.

Waiver/Release for Communicable Diseases

In consideration of being allowed to participate on behalf of Amtgard Polaris, INC and related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

IHAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY.
