Please note that new students joining after the 31st of July will be unable to participate in the end of year Showcase. This is due to costumes already being ordered.

NOTE: Exceptions can sometimes apply. Please speak to the Kreationz office if your child wishes to participate.
MID YEAR REVIEW

The Mid Year Review is a fun and laid back event which gives the students an opportunity to perform in an informal setting.

With no costumes and lots of fun, it's an event not to be missed!

COST: $12 participation fee

BONUS: Two free audience tickets are included for every student who selects a Showcase Package!

The Mid Year Reviews will be held on Saturday the 14th and Sunday the 15th of June, 2025.

SHOWCASE PARTICIPATION AGREEMENT

By selecting an option below you indicate your intention for your child to participate in the End of Year Showcase. By selecting 'Yes' you agree to the following Terms and Conditions:
- You will be available for the period of the 22nd of November to the 30th of November 2025 for rehearsals and performances.
- You must select either the Shine, Sparkle or Dream Showcase Package (see Fee Schedule in the information pack).
- You understand and consent that your child's performance will be professionally photographed and videoed, and may be used for marketing purposes.
- Students who join part way through the year and who wish to participate in the Showcase will be required to pay a one-off "catch up" payment for any Terms prior to enrolment. Eg. Enrolments in Term 2 will be required to pay the Term 1 and Term 2 Showcase package instalments.
- Should your child commence another class throughout the year, it will be assumed that your child is participating in the Showcase for that class and you will be liable for the full costume cost, unless otherwise notified at the time of enrolment.
- Should your child be unable to participate in the Showcase and we are notified NO LATER than the 30th of June 2025, your costume payments will be credited towards your next Term's fees.
- No refunds or credits will be offered should we be notified later than the 30th of June 2025. By selecting 'Yes' below you agree to be liable for the full amount of all costume purchase fees if you change your mind after the 30th of June 2025.
- Should you select 'No' and you change your mind before the 30th of June 2025, you/your child/ren may still be able to participate in the Showcase provided a costume can be ordered for them. Should this occur, the Showcase levy will be required to be paid immediately.

Yes, I acknowledge that my place in the selected classes will only be held upon payment of the non-refundable membership fee. I understand that I must complete either an authorisation of the Payrix Direct Debit form (which is available upon completion of this form) or pay my fees in full via invoice, before I can commence my classes.

PAYMENT OPTIONS

In order to hold your child's position in your selected class/es, the non-refundable and non-transferable Membership fee is payable upon completion of this form securely via credit card.

All Tuition and Showcase fees are payable in advance on a monthly, quarterly or half-yearly basis, via instalments by Payrix Direct Debit. If you do not currently have a Payrix account with Kreationz, a Direct Debit authorisation form is required to be completed once your enrolment is confirmed.

Payments can be deducted from either a financial institution or from a credit card. Please note that bank accounts attract a $0.94 fee. Credit cards attract a 2.12% plus $0.35 fee for Visa and Mastercard. A one off $2.20 data storage fee is charged for all new families.

MEMBERSHIP FEE PAYMENT

The below credit card payment is for your Membership fee and any additional uniform items you have selected. Your Tuition and Showcase fees (if applicable) will be deducted from your account upon filling out the Payrix Direct Debit authorisation at the completion of this form.

NOTE: The first tuition instalment will be deducted on the 20th of January, 2025.
