MID YEAR REVIEW

The Mid Year Review is a fun and laid back event which gives our students an opportunity to perform in an informal setting.

With lots of fun, it's an event not to be missed!

Cost: $12 participation fee

NOTE: The Mid Year Reviews will be held on Saturday the 14th and Sunday the 15th of June, 2025. Your child will only be required for one of these dates.

RECITAL PARTICIPATION AGREEMENT

Ready Set Recital is our pre-school's special end of year event. It is the highlight of our pre-school dance calendar and is not to be missed! The Recital performance is done before a small audience and Mum and/or Dad will only be a few steps away at any time - just in case someone needs a smile of encouragement or a hug.

Best of all the whole experience is about your child for the ENTIRE performance!

By selecting an option below you indicate your intention for your child to participate in the Recital. By selecting 'Yes' you agree to the following Terms and Conditions:
- An additional $42.00 per term or $16.80 per month (to total $168), will be added to your tuition for a costume (which is yours to keep), a medal and admission for four people.
- You will be available on Saturday the 6th and Sunday the 7th of December, 2025. The specific date and time for your class will be announced in early Term 4 - you will only be required for one of the above dates.
- You understand and consent that your child's performance will be photographed and videoed and may be used for marketing purposes by Kreationz Dance.
- Should your child be unable to participate in the Recital and we are notified NO LATER than the 31st of July 2025, your Recital levy will be credited towards your next Term's fees.
- No refunds or credits will be offered should we be notified later than the 31st of July 2025. By selecting 'Yes' below you agree to be liable for the full amount of the Recital fees if you change your mind after the 31st of July 2025.
- Should you select 'No' and you change your mind after the 31st of July 2025, your child may still be able to participate in the Recital, provided a costume can be ordered for them. Should this occur the Recital levy will be required to be paid immediately.
NOTE: 2025 Ready Set Recitals will take place on Saturday the 6th and Sunday the 7th of December. You will only be required for one of these dates.

Yes, I acknowledge that my place in the selected classes will only be held upon payment of the non-refundable membership fee. I understand that I must complete either an authorisation of the Payrix Direct Debit form (which is available upon completion of this form) or pay my fees in full via invoice, before I can commence my classes.

PAYMENT OPTIONS

In order to hold your child's position in your selected class/es, the non-refundable Membership fee is payable upon completion of this form securely via credit card.

All Tuition and Recital fees are payable in advance on a monthly, quarterly or half-yearly basis, via installments by Payrix Direct Debit. A Direct Debit authorisation form is required to be completed once your enrolment is confirmed.

Payments can be deducted from either a financial institution or from a credit card. Please note that bank accounts attract a $0.94 fee. Credit cards attract a 2.12% plus $0.35 fee for Visa and Mastercard. A one off $2.20 data storage fee is charged for all new families.

MEMBERSHIP FEE PAYMENT

The below credit card payment is for your Membership fee, any additional uniform items you have selected. Your Tuition and Recital fees (if applicable) will be deducted from your account upon filling out the Payrix Direct Debit authorisation at the completion of this form.

NOTE: The first tuition instalment will be deducted on the 9th of January, 2025, or within three business days of your enrolment if after this date.

PLEASE NOTE YOUR ENROLMENT IS NOT CONSIDERED FINALISED UNTIL THE DIRECT DEBIT AUTHORISATION HAS BEEN COMPLETED. YOUR CLASSES WILL NOT BE HELD WITHOUT THIS AUTHORISATION.
